<strong>Neonatal</strong> Hyperbilirubinemia-Pathologic<br />
• <strong>When</strong> to evaluate pathologic jaundice (laboratory tests):<br />
– <strong>Jaundice</strong> in first 24 hours (TcB*/TSB*)<br />
• Excessive production usually-hemolysis<br />
– TcB/TSB above 75 th percentile (high intermediate or high risk<br />
zone) or rapidly rising (i.e. crossing percentiles) on hourly<br />
nomogram (Bhutani, VK et al, 1999: 103: 6)<br />
• Cord TSB > 4-5 mg/dL or TSB > 5 mg/dL at 24 hours<br />
• TSB crossing lines (of concern) or rising more than 0.2-0.5<br />
mg/dL/hr for 4-8 hours(?)<br />
• TSB crossing lines (of concern) or increasing more than 5<br />
mg/dL per 24 hours<br />
• TSB over 13(?),15-17 ml/dL in full term infant<br />
*TcB/TSB-Transcutaneous Bilirubin/Total Serum Bilirubin
