Mechanism of action of Nitrates and nitrites<br />

These act by the formation free radical nitric oxide (NO), which interact with and activate<br />

guanylate cyclase. Nitric oxide forms a reductive nitrothiol intermediate activate a<br />

soluble cytosolic form of the enzyme guanylate cyclase and cGMP formation is thereby<br />

increased. The guanylate cyclase increases the synthesis of guanosine 3’, 5’monophosphate,<br />

which activates a protein kinase which mediates dephosphorylation of<br />

myosin responsible for the maintenance of the contractile state in smooth muscle.<br />

Mechanism of action of Calcium Channel Blockers<br />

These drugs acts by selectively inhibit calcium ion influx into heart muscle and inhibit<br />

calcium ion influx into vascular smooth muscle. It dilates the main coronary arterioles,<br />

and by inhibiting coronary artery spasm, they increase myocardial oxygen delivery in<br />

patients with Prinzmetal’s angina.<br />

Mechanism of action of β-Adrenergic Antagonist<br />

The β-Adrenergic Antagonist decreases sympathetic stimulation of the heart and thus<br />

reduces the heart rate and decreases myocardial contractibility. These effects in turn<br />

decrease the oxygen requirements of the myocardium, both during exercise and at rest.<br />

Concentrated Glyceryl Trinitrate Solution, B. P.: Concentrated glyceryl trinitrate<br />

solution is a 9-11 per cent solution of propane-1, 2, 3-triol trinitrate in ethanol (96<br />

per cent). It is a clear, colorless to pale yellow solution. Glyceryl trinitrate<br />

(nitroglycerin; nitro glycerol; trinitrin; trinitroglycerin) is the nitric acid ester of<br />

glycerin, and may be prepared by treating dehydrated glycerin with a mixture of<br />

fuming nitric acid and sulphuric acid. It is a white to pale yellow, thick,<br />

flammable, explosive liquid. It is slightly soluble in water.<br />

Glyceryl trinitrate is rapidly absorbed from the oral mucosa. It is also well<br />

absorbed from the gastrointestinal tract, but since it undergoes extensive first-pass<br />

metabolism in the liver its bioavailability is reduced. As it has short plasma half-life<br />

different long-acting formulations are available.<br />

Glyceryl trinitrate is used for the prophylaxis and treatment of stable angina. It is<br />

also indicated for congestive heart failure and may have a role in the management of<br />

myocardial infarction.<br />

Diluted Pentaerythritol Tetranitrate, I.P.<br />

Pentaerythritol tetranitrate, 2, 2-bis (hydroxymethyl) propane-1, 3- diol tetranitrate, is<br />

a white crystalline powder, practically insoluble in water. It is prepared by nitration of
