3.4 Selected primipara - episiotomy and perineal tear (L) Obstetrics, v6 Rationale Vaginal birth is the most common cause of anal sphincter injuries in women and as such obstetric anal sphincter injury is considered a major complication of vaginal birth – a complication that can have a significant impact on a woman’s quality of life. Numerator Total number of selected primipara undergoing episiotomy AND sustaining a perineal tear while giving birth vaginally. Denominator Total number of selected primipara delivering vaginally. The annual rate was 5.9 per 100 selected primipara. Variation between strata There were no significant stratum differences in 2009 and 2010. Variation between HCOs In 2010 the potential gains totalled 961 fewer perineal tears, corresponding to a reduction by approximately one half. Outliers In 2010 there were 16 outlier submissions from 12 HCOs whose combined excess was 407 more perineal tears. The outlier HCO rate was 13.5 per 100 selected primipara.
