REPORT ON THE STATE OF FOOD INSECURITY IN URBAN INDIA<br />

health care system. The town panchayats come under <strong>the</strong> service area <strong>of</strong> primary health centres<br />

and <strong>the</strong> health subcentres under <strong>the</strong>m. Municipalities (<strong>in</strong>clud<strong>in</strong>g corporati<strong>on</strong>s) with populati<strong>on</strong><br />

exceed<strong>in</strong>g 100,000 are covered under nati<strong>on</strong>al programmes like <strong>the</strong> <strong>India</strong> Populati<strong>on</strong> Project<br />

and its successors. So, <strong>the</strong>re is need for a special focus <strong>on</strong> <strong>the</strong> smaller towns and municipalities<br />

<strong>in</strong> <strong>the</strong> Nati<strong>on</strong>al <strong>Urban</strong> Health Missi<strong>on</strong>.<br />

There are some general po<strong>in</strong>ts to be kept <strong>in</strong> m<strong>in</strong>d. JNNURM and o<strong>the</strong>r urban development/<br />

urban poverty alleviati<strong>on</strong> programmes tend to emphasise <strong>the</strong> urban unit as a whole, which<br />

de facto means privileg<strong>in</strong>g <strong>the</strong> more affluent secti<strong>on</strong>s <strong>of</strong> <strong>the</strong> urban populati<strong>on</strong>. It is important<br />

to focus urban <strong>in</strong>terventi<strong>on</strong>s <strong>in</strong> JNNURM and o<strong>the</strong>r programmes <strong>on</strong> <strong>the</strong> needs <strong>of</strong> small and<br />

medium towns and <strong>on</strong> <strong>the</strong> needs <strong>of</strong> urban slums <strong>in</strong> all cities, tak<strong>in</strong>g care to address <strong>the</strong> needs<br />

<strong>of</strong> <strong>the</strong> poor with regard to shelter, water, sanitati<strong>on</strong>, dra<strong>in</strong>age and nutriti<strong>on</strong> educati<strong>on</strong>. It is a<br />

matter <strong>of</strong> political will that adequate resources be found through <strong>the</strong> regular fiscal mechanisms<br />

to undertake <strong>the</strong> necessary <strong>in</strong>frastructure <strong>in</strong>vestments <strong>in</strong> urban areas that address <strong>the</strong>se basic<br />

needs. It is also important to ensure that PDS, ICDS and MDMS reach <strong>the</strong> poor effectively.<br />

The elected urban local bodies need to be <strong>in</strong>vested with both power and resp<strong>on</strong>sibility for<br />

design<strong>in</strong>g and implement<strong>in</strong>g all food and nutriti<strong>on</strong> programmes, and provided <strong>the</strong> necessary<br />

f<strong>in</strong>ancial support by <strong>the</strong> higher levels <strong>of</strong> government, especially <strong>the</strong> Central government.<br />

To ensure effective devoluti<strong>on</strong>, <strong>the</strong> capabilities <strong>of</strong> local bodies need to be streng<strong>the</strong>ned<br />

c<strong>on</strong>siderably through appropriate programmes <strong>of</strong> capacity build<strong>in</strong>g. Elected urban local bodies<br />

could be encouraged to <strong>in</strong>volve local communities <strong>in</strong> promot<strong>in</strong>g hygiene and sanitati<strong>on</strong>, with<br />

<strong>the</strong> voluntary support <strong>of</strong> <strong>the</strong> numerous community organisati<strong>on</strong>s work<strong>in</strong>g <strong>on</strong> <strong>the</strong>se issues.<br />

Educati<strong>on</strong>, social mobilisati<strong>on</strong> and regulati<strong>on</strong> will all be needed.<br />

A Nutriti<strong>on</strong> Literacy Movement should be launched and home gardens promoted, wherever<br />

this is feasible, based <strong>on</strong> <strong>the</strong> pr<strong>in</strong>ciple <strong>of</strong> f<strong>in</strong>d<strong>in</strong>g a horticultural remedy for every nutriti<strong>on</strong>al<br />

malady (with particular reference to micr<strong>on</strong>utrient deficiencies).<br />

Based <strong>on</strong> <strong>the</strong> recent experience with food prices <strong>in</strong>flati<strong>on</strong>, it will be desirable to promote<br />

c<strong>on</strong>sumer cooperatives <strong>in</strong> urban areas to m<strong>in</strong>imise <strong>the</strong> very wide gap between wholesale and<br />

retail prices. C<strong>on</strong>sumer cooperatives can be a supplement to fair price shops.<br />

The m<strong>in</strong>imum support price announced for a number <strong>of</strong> crops is be<strong>in</strong>g implemented <strong>on</strong>ly <strong>in</strong> <strong>the</strong><br />

case <strong>of</strong> wheat and rice. It is necessary to broaden <strong>the</strong> food basket by <strong>in</strong>clud<strong>in</strong>g nutritious millets,<br />

legumes and tubers. Provid<strong>in</strong>g appropriate remunerative prices for <strong>the</strong>se crops will help expand<br />

<strong>the</strong>ir producti<strong>on</strong> PDS can cover o<strong>the</strong>r nutritious foods, <strong>in</strong> additi<strong>on</strong> to basic staples.<br />

The need to l<strong>in</strong>k nutriti<strong>on</strong> with disease management is particularly important <strong>in</strong> cases like<br />

tuberculosis, HIV/AIDS, leprosy, etc., where a drug-based approach al<strong>on</strong>e will not help to cure<br />

<strong>the</strong> patient. A food-cum-drug based approach will be important.<br />

F<strong>in</strong>ally, urban food security is not a matter <strong>of</strong> urban policy al<strong>on</strong>e. It is bound up with <strong>the</strong> urban<br />

ec<strong>on</strong>omic structure characterised by a high degree <strong>of</strong> <strong>in</strong>equality and also with <strong>the</strong> impact <strong>of</strong><br />
