Antithrombotic therapy
Tissue plasminogen activator (tPA)
• Produced by human vascular endothelium.
• Activates plasminogen (inactive precursor to
plasmin).
• Levy, J Reconst Microsurg 1991 - no difference
between tPA and urokinase in the rat model.
• Others report cases of free flap salvage with tPA
infusion - Fedem & Walton, J Reconst Microsurg
1989. Arnljots, PRS 1992. Romano, 1991.
• Reports on selective tPA infusion.
Antithrombotic therapy
• Davies, Br J Plast Surg 1982 - surveyed the use of
anticoagulation in clinical microsurgery.
» 73 centers in 22 countries.
» Found equal success rates (89%) for free flaps with
anticoagulation (n=691) and flaps without
anticoagulation (n=134).
» For limb replantation - success rat was lower with
anticoagulation (76%) than without (89%).
