Changed sex ana<strong>to</strong>my<br />

Ana<strong>to</strong>mically, a <strong>trans</strong> man may or may not have: a vagina, a micro-penis, a penis<br />

with erectile capacity. Having a penis with erectile capacity means that you are less<br />

likely <strong>to</strong> have a vagina, however such a combination is not impossible<br />

All these possibilities will impact on sexual behaviours and, consequently, the<br />

degree of risk of STIs <strong>to</strong> which a <strong>trans</strong> man may be exposed. You will be at least<br />

risk if you are having non-penetrative sex (most likely with a woman). However,<br />

infections can be passed between partners through rubbing vulvas <strong>to</strong>gether or<br />

hand contact. You will be at most risk if you are having anal penetrative sex (most<br />

likely with a man); and there are various shades of risk in between. Penetration<br />

with sex <strong>to</strong>ys where these are shared can also cause infections <strong>to</strong> be passed<br />

between partners.<br />

The behaviours that put you at most risk are:<br />

Unprotected penetrative sex, especially anal sex; and<br />

Multiple sexual partners<br />

Hygiene is important although, on its own, it will not prevent infections being<br />

passed on. You should have clean hands and clean genitals, sex <strong>to</strong>ys should be<br />

washed every time they are used, and this includes when they are used <strong>for</strong> more<br />

than one orifice on the same occasion. Avoid oral sex if you have any cuts or sores<br />

on the mouth; use latex condoms <strong>for</strong> penile penetration. Wear latex gloves and use<br />

plenty of water-based lubricant <strong>for</strong> vaginal and anal fisting, or when using dildos<br />

(water-based products do not damage rubber).<br />

Douching should not be necessary as the vagina is self-cleansing. Soreness<br />

around the entrance <strong>to</strong> the vagina can occur when using perfumed soaps and<br />

bubble baths. Specific conditions such as Bacterial Vaginosis and Thrush can also<br />

be caused in this way, and it is also possible that they can be passed on through<br />

sexual <strong>to</strong>uching or using sex <strong>to</strong>ys. However, they are not necessarily related <strong>to</strong><br />

sexual activity. The symp<strong>to</strong>ms of Bacterial Vaginosis are a fishy smelling thin green<br />

discharge; this is treatable with antibiotics. The symp<strong>to</strong>ms of Thrush include:<br />

itching; pain on vaginal penetration; burning sensation when passing urine and<br />

thick white discharge. Medicated creams, pessaries and tablets can be bought at<br />

pharmacies but you should see your doc<strong>to</strong>r if these symp<strong>to</strong>ms persist.<br />
