Additionally, over 2 million work days are lost each year in the United States as a result of postphlebitic syndrome. 5 — Similar studies among individuals with PAOD have not been done; however, peripheral arterial disease, arterial insufficiency, and arterial ulcers also result in lifestyle changes. Therefore, similar psychological, sociological, and financial conditions are the most likely outcomes. REVIEW OF THE ARTERIAL SYSTEM • Arterial flow moves from the heart downward (Figure 5–1). • Lower extremity arterial perfusion is initiated with satisfactory cardiac performance. — First arterial collateral vessels of importance to leg perfusion are the intercostal arteries arising from the descending thoracic aorta. — They are the only collaterals in distal aortic occlusion. • The greatest reduction in size of the aorta occurs distal to the renal arteries. — Lumbar arteries are usually paired vessels at each vertebral level in the abdomen and are important to lower extremities as collateral pathways when the patient has severe aorto-iliac occlusive disease or distal aortic occlusions. — Internal iliac arteries through the gluteal and pudental branches are another collateral pathway to leg perfusion. — The external iliac artery at the level of the inguinal ligament becomes the common femoral artery. This vessel bifurcates into the deep femoral and superficial femoral arteries.
