• The clinical provider requires assistance in managing a particularly<br />

challenging wound, patient, caregiver, and situation.<br />

— CWOCN/ET/CNS<br />

<strong>Wound</strong> care is not effective in expected time period.<br />

Products are not efficacious, too difficult for patient or<br />

caregiver to manage, too expensive, or not available.<br />

Required wound care can only be provided by CWOCN<br />

as in the case of conservative or sharp debridement.<br />

Questions concerning type and amount of pressure<br />

reduction devices.<br />

— MSW: to evaluate the patient or caregiver who is unwilling<br />

or unable to participate in the wound care as developed<br />

in the POC.<br />

Request the MSW to determine if there are any psychological<br />

or sociological reasons for this inability or<br />

unwillingness.<br />

Request the MSW to assist in developing a POC for all<br />

other providers to reach the expected and planned for<br />

outcomes. This includes a plan to manage health and<br />

other patient/caregiver behaviors.<br />

Request MSW to assist when there are financial concerns<br />

of the patient or caregiver relative to the POC such<br />

as the ability to pay copayments.<br />

— Occupational Therapist (OT)<br />

Whenever there is a question about the patient or caregiver’s<br />

potential for self-care of the wound or independence<br />

in other activities of daily living (ADLs).<br />

When there are questions concerning the amount of<br />

energy conservation needed in a specific situation.<br />

When there are questions concerning adaptive equipment<br />

to further the patient’s independence.<br />

— Physical Therapy (PT)<br />

Whenever there are questions concerning the patient's<br />

mobility and independent function relative to mobility.
