24 <strong>Social</strong> <strong>determ<strong>in</strong>ants</strong> <strong>of</strong> <strong>health</strong> <strong>in</strong> <strong>countries</strong> <strong>in</strong> <strong>conflict</strong><br />

10 000 per day was 2.6 and 5.9, respectively, represent<strong>in</strong>g, over a period <strong>of</strong> 4 months,<br />

1525 excess deaths among the 57 600 people <strong>in</strong> the d<strong>is</strong>trict. Most <strong>of</strong> the child deaths were<br />

due to diarrhoea, respiratory tract <strong>in</strong>fections, measles and scurvy, reflect<strong>in</strong>g underly<strong>in</strong>g<br />

malnutrition. Th<strong>is</strong> study was conducted by Save the Children, USA [51].<br />

Maternal mortality ratios <strong>in</strong> Afghan<strong>is</strong>tan are among the highest <strong>in</strong> the world, due<br />

to a comb<strong>in</strong>ation <strong>of</strong> pers<strong>is</strong>tent poverty and <strong>conflict</strong>, at approximately 1600 per 100 000<br />

live births <strong>in</strong> 2002 [52]. Accord<strong>in</strong>g to a 2000–2002 reproductive age mortality study,<br />

figures ranged from 418 <strong>in</strong> Kabul to 6507 <strong>in</strong> Ragh d<strong>is</strong>trict, Badakshan prov<strong>in</strong>ce, the<br />

highest maternal mortality ratio ever recorded. Even though Ragh d<strong>is</strong>trict was not directly<br />

affected by <strong>conflict</strong>, it was affected by the general paucity <strong>of</strong> <strong>health</strong> services found <strong>in</strong><br />

Afghan<strong>is</strong>tan. It <strong>is</strong> <strong>in</strong> a remote region <strong>in</strong> the H<strong>in</strong>du Kush mounta<strong>in</strong>s, up to 10 days’ ride<br />

or walk from the nearest hospital with emergency obstetric care. Given the high total<br />

fertility rates, these figures translate <strong>in</strong>to a total lifetime r<strong>is</strong>k <strong>of</strong> maternal death <strong>of</strong> 1 <strong>in</strong><br />

42 <strong>in</strong> Kabul and 1 <strong>in</strong> 3 <strong>in</strong> Ragh d<strong>is</strong>trict. On the bas<strong>is</strong> <strong>of</strong> such figures, Afghan<strong>is</strong>tan can be<br />

considered the worst place <strong>in</strong> the world to become pregnant [53,54,55].<br />

Table 2 provides a summary <strong>of</strong> the major d<strong>is</strong>eases reported to the Regional Office<br />

from <strong>conflict</strong> <strong>countries</strong>. Rates <strong>of</strong> HIV prevalence per 100 000 population (15 and over)<br />
