<strong>Social</strong> <strong>determ<strong>in</strong>ants</strong> <strong>of</strong> <strong>health</strong> <strong>in</strong> <strong>countries</strong> <strong>in</strong> <strong>conflict</strong> 51<br />

The Palest<strong>in</strong>ian Medical Relief Society supports a programme for targeted<br />

populations liv<strong>in</strong>g <strong>in</strong> extreme poverty and suffer<strong>in</strong>g from a lack <strong>of</strong> food and economic<br />

resources due to closures and the restricted mobility <strong>of</strong> people and goods. The Society<br />

mobilizes its resources to reach those liv<strong>in</strong>g <strong>in</strong> the most remote, <strong>is</strong>olated and poor<br />

regions, which are not be<strong>in</strong>g provided with services from the national authority or<br />

other organizations. It does not rely only on food prov<strong>is</strong>ion to tackle malnutrition, as<br />

th<strong>is</strong> approach has created problems <strong>of</strong> dependency. In order to tackle malnutrition and<br />

anaemia <strong>in</strong> a comprehensive manner, the programme first screened affected families<br />

and provided medical treatment and vitam<strong>in</strong>s. It then provided job creation activities for<br />

target households and <strong>in</strong>dividuals liv<strong>in</strong>g below the poverty l<strong>in</strong>e, pay<strong>in</strong>g them for their<br />

labour. Job creation programmes focused on develop<strong>in</strong>g community assets by build<strong>in</strong>g<br />

schools, cl<strong>in</strong>ics and k<strong>in</strong>dergartens. Support for female-headed households was also<br />

provided, work<strong>in</strong>g with organizations already active <strong>in</strong> th<strong>is</strong> field. The Palest<strong>in</strong>ian Medical<br />

Relief Society provided <strong>in</strong>dividuals with sheep and goats, a source <strong>of</strong> milk and cheese<br />

for households, a valuable source <strong>of</strong> nutrition for the household. It also ass<strong>is</strong>ted women<br />

to enter the market and sell their products [112].<br />

The Gaza Community Mental Health Programme began <strong>in</strong> 1990. Dur<strong>in</strong>g the<br />

second <strong>in</strong>tifada the <strong>conflict</strong> rapidly escalated and there was a grow<strong>in</strong>g need for mental<br />

<strong>health</strong> ass<strong>is</strong>tance as a result <strong>of</strong> the <strong>in</strong>creased prevalence <strong>of</strong> mental d<strong>is</strong>orders among the<br />

population <strong>in</strong>creas<strong>in</strong>gly exposed to trauma, violence and torture. These acts <strong>of</strong> violence<br />

d<strong>is</strong>turbed family members and entire communities, neighbourhoods, schools and<br />

children. A new approach to mental <strong>health</strong>, with a greater community orientation that<br />

directly addressed dysfunctions <strong>in</strong> the social environment was identified, mov<strong>in</strong>g away<br />

from the <strong>in</strong>stitutional therapeutic approaches then available through the M<strong>in</strong><strong>is</strong>try <strong>of</strong><br />

Health. Human rights considerations for community members were the reference po<strong>in</strong>ts<br />

for organiz<strong>in</strong>g the programme and for deliver<strong>in</strong>g services. Because violence <strong>is</strong> felt at all<br />

levels <strong>in</strong> the community, the programme aims to simultaneously address many sectors <strong>of</strong><br />

the society. It aims to give tools to the community to enable it to face future problems <strong>in</strong><br />

the long term. The programme works with the M<strong>in</strong><strong>is</strong>try <strong>of</strong> Health and UNRWA to deliver<br />

complementary services, meet<strong>in</strong>g regularly to assess and coord<strong>in</strong>ate cr<strong>is</strong><strong>is</strong> <strong>in</strong>terventions.<br />

Thirty per cent <strong>of</strong> the effort has focused on deliver<strong>in</strong>g cl<strong>in</strong>ical services for people <strong>in</strong> need.<br />

Tra<strong>in</strong><strong>in</strong>g and research engage up to 70% <strong>of</strong> the programme, which <strong>is</strong> seen as essential<br />

for gather<strong>in</strong>g evidence and for capacity-build<strong>in</strong>g [67,68,89,112].<br />

Protection aga<strong>in</strong>st sexual violence <strong>in</strong> Darfur has focused on l<strong>is</strong>ten<strong>in</strong>g to women,<br />

female empowerment and provid<strong>in</strong>g protection for r<strong>is</strong>ky activities. Respond<strong>in</strong>g to<br />

concerns voiced <strong>in</strong> the civil society report about sexual violence, local nongovernmental<br />

organizations can and do support <strong>in</strong>dividual and small group activities, but must have the<br />

support <strong>of</strong> <strong>in</strong>ternational agencies with a responsibility to ma<strong>in</strong>ta<strong>in</strong> security and human<br />

rights.<br />
