A social <strong>determ<strong>in</strong>ants</strong> approach to the<br />
<strong>health</strong> impact <strong>of</strong> <strong>conflict</strong><br />
5.1 Major social <strong>determ<strong>in</strong>ants</strong> <strong>in</strong> <strong>conflict</strong> sett<strong>in</strong>gs<br />
The two major structural <strong>determ<strong>in</strong>ants</strong>, reflect<strong>in</strong>g the character <strong>of</strong> armed <strong>conflict</strong> <strong>in</strong><br />
the twenty-first century, are the loss <strong>of</strong> human rights and breaches <strong>of</strong> medical neutrality.<br />
A major <strong>in</strong>termediate determ<strong>in</strong>ant <strong>is</strong> psychosocial d<strong>is</strong>tress result<strong>in</strong>g from the structural<br />
<strong>determ<strong>in</strong>ants</strong>, expressed <strong>in</strong> terms <strong>of</strong> a f<strong>in</strong>al <strong>health</strong> outcome—mental <strong>health</strong> problems.<br />
Thematic areas can be explored as guides to action and <strong>in</strong>terventions (see Box 1).<br />
The loss <strong>of</strong> human rights<br />
The loss <strong>of</strong> human rights <strong>is</strong> the first and most important <strong>of</strong> the major social<br />
<strong>determ<strong>in</strong>ants</strong> <strong>in</strong> <strong>conflict</strong> sett<strong>in</strong>gs. Human rights losses are also similar to those<br />
character<strong>is</strong>tics that identify a “humanitarian cr<strong>is</strong><strong>is</strong>” that requires <strong>in</strong>ternational<br />
<strong>in</strong>tervention (see Section 3.1). The right to live <strong>in</strong> dignity and security are enshr<strong>in</strong>ed <strong>in</strong><br />
UN conventions and protocols. They are, as Amartya Sen rem<strong>in</strong>ds us, primarily ethical<br />
demands that make claims on <strong>in</strong>dividuals and organizations that are capable <strong>of</strong> tak<strong>in</strong>g<br />
action to guarantee such rights.<br />

Structural <strong>determ<strong>in</strong>ants</strong> <strong>in</strong>clude:<br />
• the loss <strong>of</strong> human rights which are usually addressed dur<strong>in</strong>g emergency<br />
responses but which also need to be addressed <strong>in</strong> the long term through build<strong>in</strong>g<br />
up capacities;<br />
• breaches <strong>of</strong> medical neutrality which require national and <strong>in</strong>ternational action<br />
and the <strong>in</strong>volvement <strong>of</strong> nongovernmental organizations.<br />
Intermediate <strong>determ<strong>in</strong>ants</strong> <strong>in</strong>clude:<br />
• stress, d<strong>is</strong>tress and d<strong>is</strong>ease: tackl<strong>in</strong>g mental <strong>health</strong> problems <strong>in</strong> the social<br />
sett<strong>in</strong>g rather than through <strong>in</strong>dividual counsell<strong>in</strong>g.<br />
Thematic areas as guides to action <strong>in</strong>clude:<br />
• vulnerable groups: everyone <strong>is</strong> vulnerable;<br />
• loss <strong>of</strong> livelihood;<br />
• loss <strong>of</strong> community support and social networks;<br />
• prov<strong>is</strong>ion <strong>of</strong>, and access to, <strong>health</strong> services.<br />
