Donations of Vacc<strong>in</strong>e, 1958-1 966<br />
Apart from encourag<strong>in</strong>g <strong>the</strong> establishment of national <strong>smallpox</strong> eradication pro-<br />
grammes, <strong>the</strong> only important consequence of <strong>the</strong> resolution on global <strong>smallpox</strong> eradication<br />
adopted by <strong>the</strong> Twelfth World Health Assembly <strong>in</strong> 1959 (see Chapter 9) was <strong>the</strong> <strong>in</strong>itiation<br />
of donations of <strong>smallpox</strong> <strong>vacc<strong>in</strong>e</strong>, both to WHO <strong>and</strong> on a larger scale <strong>in</strong> bilateral assistance<br />
programmes. Between 1958 <strong>and</strong> 1966 <strong>the</strong> <strong>vacc<strong>in</strong>e</strong> donated to WHO totalled 47 062 500<br />
doses (from Jordan, Madagascar, Mexico, <strong>the</strong> Ne<strong>the</strong>rl<strong>and</strong>s, <strong>the</strong> Philipp<strong>in</strong>es, Switzerl<strong>and</strong>,<br />
Thail<strong>and</strong>, <strong>the</strong> USSR <strong>and</strong> <strong>the</strong> United K<strong>in</strong>gdom). Of this amount, 25 million doses of <strong>vacc<strong>in</strong>e</strong><br />
had been pledged by <strong>the</strong> government of <strong>the</strong> USSR <strong>in</strong> 1958 when it proposed that WHO<br />
should undertake a global <strong>smallpox</strong> eradication programme ; <strong>the</strong>y were delivered between<br />
1960 <strong>and</strong> 1964. In bilateral donations between 1961 <strong>and</strong> 1966 <strong>the</strong> USSR also provided some<br />
700 million doses of <strong>vacc<strong>in</strong>e</strong>, which met <strong>the</strong> annual requirements of Afghanistan, Burma,<br />
India <strong>and</strong> some o<strong>the</strong>r countries dur<strong>in</strong>g this period.<br />
Shortfalls <strong>in</strong> Vacc<strong>in</strong>e-Quality <strong>and</strong><br />
Quantity<br />
S<strong>in</strong>ce, although <strong>smallpox</strong> <strong>vacc<strong>in</strong>e</strong> had<br />
been <strong>in</strong> use s<strong>in</strong>ce 1800, st<strong>and</strong>ardized <strong>vacc<strong>in</strong>e</strong><br />
production procedures <strong>and</strong> reliable assay<br />
methods for quality control were not de-<br />
veloped until <strong>the</strong> 1950s (see Chapter 7), <strong>the</strong><br />
quality of <strong>the</strong> <strong>vacc<strong>in</strong>e</strong> varied significantly <strong>in</strong><br />
different countries. Although <strong>the</strong> quality of<br />
<strong>vacc<strong>in</strong>e</strong>s donated to WHO had been tested<br />
s<strong>in</strong>ce 1959, until 1967 such <strong>vacc<strong>in</strong>e</strong>s account-<br />
ed for an average of only 7 million doses per<br />
year (Table 11.1), or less than 2% of <strong>the</strong><br />
<strong>vacc<strong>in</strong>e</strong> needed <strong>in</strong> endemic countries. The<br />
rema<strong>in</strong><strong>in</strong>g <strong>vacc<strong>in</strong>e</strong> was provided by domestic<br />
production or through bilateral aid pro-<br />
grammes <strong>and</strong> by a number of producers <strong>who</strong><br />
sold <strong>vacc<strong>in</strong>e</strong> to <strong>the</strong> endemic countries. The<br />
titre of most of <strong>the</strong>se <strong>vacc<strong>in</strong>e</strong>s was unknown<br />
to WHO <strong>and</strong>, even when known, <strong>the</strong> tests<br />
used to determ<strong>in</strong>e it were of uncerta<strong>in</strong> reli-<br />
ability <strong>and</strong> conducted ma<strong>in</strong>ly by <strong>the</strong> produc-<br />
tion laboratories <strong>the</strong>mselves. In many labora-<br />
tories, <strong>the</strong> potency was tested by vacc<strong>in</strong>at<strong>in</strong>g<br />
10 children <strong>and</strong> determ<strong>in</strong><strong>in</strong>g <strong>the</strong> proportion<br />
of takes, or by us<strong>in</strong>g <strong>the</strong> rabbit sk<strong>in</strong> scarifica-<br />
tion method, which was imprecise; <strong>in</strong> addi-<br />
tion, few laboratories tested <strong>the</strong> <strong>vacc<strong>in</strong>e</strong> for<br />
heat stability.<br />
Thus WHO was faced, not with <strong>the</strong> rela-<br />
tively small problem of clos<strong>in</strong>g a gap between<br />
<strong>the</strong> total requirement <strong>and</strong> supplies obta<strong>in</strong>ed<br />
from exist<strong>in</strong>g <strong>vacc<strong>in</strong>e</strong> donors <strong>and</strong> local pro-<br />
duction of <strong>vacc<strong>in</strong>e</strong> <strong>in</strong> <strong>the</strong> endemic countries,<br />
but with <strong>the</strong> much larger task of develop<strong>in</strong>g a<br />
production-donation system to serve most of<br />
<strong>the</strong> endemic countries <strong>and</strong> <strong>the</strong>ir neighbours.<br />

As is shown <strong>in</strong> Table 11.1, <strong>vacc<strong>in</strong>e</strong> dona-<br />
tions to WHO were substantially <strong>in</strong>creased<br />
after <strong>the</strong> Intensified Smallpox Eradication<br />
Programme was <strong>in</strong>itiated <strong>in</strong> 1967. In <strong>the</strong> early<br />
days of <strong>the</strong> programme, one of <strong>the</strong> difficulties<br />
<strong>in</strong> <strong>in</strong>creas<strong>in</strong>g <strong>the</strong> amounts donated was that<br />
laboratories produc<strong>in</strong>g <strong>smallpox</strong> <strong>vacc<strong>in</strong>e</strong> <strong>in</strong><br />
most countries had only a limited capacity<br />
that was barely enough to meet <strong>the</strong>ir national<br />
programme requirements, let alone help to<br />
supply <strong>the</strong> very much larger amounts of<br />
<strong>vacc<strong>in</strong>e</strong> needed for a global eradication<br />
programme.
