ADMINISTRATIVE PENALTY AND DURATION OF SUSPENSION<br />
TYPE OF VIOLATION<br />
a. Failure to produce inspection<br />
records or related work<br />
orders to the <strong>Department</strong>, or<br />
agent on request.<br />
b. Failure to maintain inspection<br />
log, or improper, inaccurate<br />
or incomplete recording <strong>of</strong><br />
information on inspection<br />
records.<br />
c. Failure to assign correct<br />
expiration/date month on<br />
certificate <strong>of</strong> inspection.<br />
d. Failure to conspicuously<br />
display inspection station<br />
license, hourly rate or flat fee<br />
rate.<br />
e. Failure to notify the<br />
<strong>Department</strong> immediately in<br />
writing <strong>of</strong> changes <strong>of</strong><br />
ownership, name or location<br />
affecting an Official<br />
<strong>Inspection</strong> Station.<br />
f. Failure to report within two (2)<br />
business days <strong>of</strong> the loss or<br />
theft <strong>of</strong> certificate <strong>of</strong><br />
inspection to the <strong>Department</strong>.<br />
g. Failure to immediately notify<br />
the <strong>Department</strong> <strong>of</strong> Motor<br />
Vehicles upon temporary or<br />
permanent closing <strong>of</strong> the<br />
inspection station or a<br />
change <strong>of</strong> business hours.<br />
