On 7/6/11, DEP met with the UCHD on site; there was no sewage spill so UCHD left the rest <strong>of</strong><br />

the compliance issues to DEP. DEP sent an e-mail to the owner <strong>of</strong> the plaza outlining what<br />

needs to be done to the SSTS. On 7/11/11, DEP field staff inspected the repair <strong>of</strong> the pumps.<br />

DEP performed site visits on 7/18 and 8/2/11; septic failure was not observed. DEP sent an email<br />

to the owner to remind him that a few items, including total flows to the existing SSTS,<br />

need to be addressed. DEP received an e-mail from the property owner on 10/7/11. DEP<br />

received an e-mail from the owner on 10/18/11; he has directed the contractor to call DEP when<br />

the work is to be done. DEP received a call on 11/4/11 from the contractor, who has installed a<br />

traffic rated riser and ring on the grease trap behind the pizza shop.<br />

permit from DEC on 4/6/11. DEP performed a site visit on 5/5/11. DEP received DEC's<br />

notification <strong>of</strong> availability for review for the increase through-put, extend hours, process outside<br />

due to flood debris from DEC on 12/21/11.<br />
