Otoacoustic Emissions (OAEs)
Otoacoustic emissions are sounds that are produced by the
cochlea (outer hair cells) and can be measured in the ear canal.
When sound passing through the ear canal reaches the cochlea,
the vibration stimulates thousands of tiny hair cells. This creates
a byproduct that can be detected and measured: otoacoustic
emissions. OAEs only occur in a normal cochlea with normal
hearing sensitivity. If there is damage to the outer hair cells,
which produces hearing loss, then OAEs will not be present. In
general, OAEs will be present if hearing is at 30 dB or better.
ERO•SCAN
Why are OAEs Clinically Valuable?
OAEs are widely applied in pediatric and adult patient populations
for a variety of reasons. OAEs are an index of outer hair cell activity.
Because of their dependence on normal cell metabolism, almost all
insults to the cochlea first affect the outer hair cells. For example,
vascular, noise damage or hypoxic cochlear deficits will be invariably
reflected by reduced OAE amplitude. Therefore, assuming normal
middle ear function, OAE abnormalities provide early and compelling
evidence of cochlear (outer hair cell) dysfunction. Additional clinical
advantages of OAE are:
• Brief test time: Usually less than a minute per ear
• Relatively simple technique: Little training is required
• Objective: Unaffected by attention, cognition, cooperation
• Independent of age: OAEs can even be recorded from
newborn infants
• Ear specific: Test results for each ear
• Frequency specific: Information for many individual frequencies
• Test up to 10 frequency bands as close as 100 Hz apart
Clinical Applications of OAEs
Given their sensitivity to cochlear dysfunction, and the clinical
advantages, it’s not surprising that OAEs are useful in auditory
assessment of diverse patient populations. One of the most
common applications of OAEs is screening persons at risk for
hearing impairment. OAE screening outcome is generally
described as either “Pass” or “Refer.” A pass outcome is
reported when OAEs are present (≥ 6 dB above the noise floor)
for the majority of test frequencies. Although the presence of
OAEs does not always indicate normal hearing sensitivity, a
pass outcome rules out serious degrees of hearing loss. A refer
OAE screening outcome should be viewed as a clear risk factor
for hearing loss that could affect communication. Patients who
yield a refer outcome for OAE screening should be referred for
diagnostic hearing assessment, and possible audiological or
medical management.
OAE Say Hello to Tympanometry
Ero•Scan Pro
As a non-invasive test, tympanometry can quickly and easily be performed on
patients of any age, from infants to adults. The objectivity of tympanometry
means the results are not dependant upon a response from the patient.
Advantages of Tympanometry:
• Middle Ear evaluations
• Objective test method
• Test patients of any age
• Documented test results
Features of Ero Scan Pro:
• Print to thermal paper, PDF, or 8.5 x 11 PC printer
• Customizable settings and protocols
• Optional patient management database
• Sequenced protocols permit a full test battery
EroScan Pro Screen Shots
The EroScan Pro is the only system that has
the capability of linking DP & TE protocols for
diagnostic cochlear mapping.
The only OAE system that has the option of
adding tympanometry in both 226Hz and
1000Hz, probe tones.
DPOAE/ TEOAE/TYMP
• OAE and Tympanometry are completed in as little
as 20 seconds
• Intuitive interface for fast, easy operation
• Advanced diagnostic capabilities
• One probe fit, three tests; (CPT codes) 92567, 92587 &
92588. Run three tests without taking probe out of the
patient’s ear
Portability
The EroScan Pro hand-held unit runs on batteries and allows
you to move from room to room. The probe also makes it
easy to maneuver around the head of your patient to attain
a tight ear seal.
Reimbursement
Otoacoustic emissions are reimbursable using the Current
Procedural Terminology (CPT) codes. To date, health care
reimbursement has varied in terms of cost per test but no
problems have arisen if the appropriate codes are utilized.
The most typical CPT code for OAE screening is:
92587:
Evoked otoacoustic emissions; limited (single stimulus
level, either transient or distortion product). This is the most
typical code and would be considered a screening code or
sampling of the cochlea.
92588:
Comprehensive or diagnostic evaluation (comparison of
transient and/or distortion product otoacoustic emissions
at multiple levels and frequencies). This is for diagnostic
OAE testing, or mapping of the cochlea.
