including Medicaid accepted. Sliding fee scale based on household income available. Services include<br />

Home visitation for support and education to high-‐risk pregnant women and their children up to age<br />
5 are provided by public health nurses and a social worker. Two lactation consultants are on staff for<br />
breastfeeding support and education. GCCHD also provides routing childhood vaccinations for<br />
children ages 1-‐18 years for a donation <strong>of</strong> $15 per vaccine given. No child is denied service for<br />
immunizations due to inability to pay.<br />

CHIP is a low-‐cost, private health insurance plan that provides coverage for eligible <strong>Montana</strong> children<br />
up to age 19. Parents are in charge <strong>of</strong> the health care their children receive, and in some cases they<br />
are expected to make small co-‐payments.<br />
To be eligible, a child must:<br />
Be a <strong>Montana</strong> resident and U.S. citizen or qualified alien.<br />
Have been without health insurance coverage for at least three months.<br />
Be ineligible for Medicaid.<br />
Meet income guidelines.<br />
Have parents who do not work for the State <strong>of</strong> <strong>Montana</strong>.<br />
