<strong>Gumala</strong> <strong>Aboriginal</strong> <strong>Corporation</strong><br />
The Member is required to provide documentation of their<br />
appointment date and time (e.g. confirmation of appointment, letter from<br />
physician or health professional, etc).<br />
Please note: If the Member uses the travel, accommodation<br />
and food assistance but does not attend their<br />
appointment future assistance may not be<br />
considered.<br />
Should the application be approved the following will apply:<br />
Travel<br />
2.1.12.1 When applying for flights or fuel to attend an appointment<br />
the Member should make every effort to apply through<br />
PATS first (this should be listed on your application form)<br />
2.1.12.2 Fuel will not be paid to attend a General Practitioner<br />
(Specialist doctor’s appointments only, amount of fuel<br />
approved will reflect the distance to be travelled and will be at<br />
GAC Discretion)<br />
Accommodation<br />
2.1.12.3 When applying for accommodation to attend an<br />
appointment the Member should make every effort to<br />
apply through PATS first (this should be listed on your<br />
application form)<br />
2.1.12.4 Accommodation will only be paid for the period required<br />
to attend the Specialists appointment, taking into account<br />
the time of the appointment and travel requirements (e.g.<br />
If the appointment is at 2pm the Member will be unable to make a<br />
4.30pm flight and so will be required to travel the following day).<br />
Food<br />
2.1.12.5 When applying for meals to attend an appointment the<br />
Member should make every effort to apply through PATS<br />
first (this should be listed on your application form)<br />
2.1.12.6 Meals will only be paid for the period required to attend<br />
the appointment, taking into account the time of the<br />
appointment and travel requirements (e.g. If the appointment<br />
is at 2pm the Member will be unable to make a 4.30pm flight and so<br />
will be required to travel the following day).<br />
2.1.12.7 Meals will only be supplied to Members and/or their<br />
children as approved by <strong>Gumala</strong><br />
