crepancy in firmness between the affected and nonaffected sides is often mild enough to not require any treatment. The patients are often satisfied regardless. Like any other complication, however, there are rare cases of severe encapsulation requiring surgical correction. Statistical risks of this complication vary widely within the professional literature. Again, I can only express my own experience over a reasonable length of time and volume of cases, and report that I have encountered very few instances of capsular contracture. Another element of pre-operative discussion between the patient and the doctor involves the choice between saline and silicone implants. A complete historical discussion of everything that has been written on the subject is clearly beyond the scope of this article, although my patients and I do, at times, digress on this matter for quite a while at consultation. The following few points are, again, my well-informed impressions. Almost every patient can have either type of implant. Neither of them has been scientifically shown to cause specific disease; if they did, I doubt the FDA would release them to the public. Although they look quite similar in the body, the silicone implant feels significantly softer. This is of particular value in a very thin individual. The disadvantage of the silicone implant is that it is somewhat more difficult to promptly detect a deflation than it is with a saline, although, again, the situation does not seem to directly cause illness, regardless. It is my practice to discuss these matters at length with each patient and let her decide what seems right for her. Among the final issues often discussed at consultation is the choice of size. Although the final decision is in the hands of the patient, since it is ultimately she who needs to be satisfied, I often begin by reminding the patient that we are considering doing the same operation and undertaking the same set of risks for an implant of any size. Although my own personal sensibility runs toward more modest (less showy) implants, it has been my experience that most women who are interested in breast augmentation seem to want to be a little fuller than that. Most women of average size, average age, and average sensibility routinely want to be a C-cup. A “B” may be a little disappointing, and a “D” is often extreme, so somewhere in the Ccup range is often where we end up. A full separate consultation, complete with representative photographs, is often required to complete this discussion pre-operatively. Despite the aforementioned issues and potential complications, breast augmentation remains the most requested, most predictable, and most universally satisfying surgical procedure in my (and other plastic surgeons’) practice.
