Health & Fitness Membership
Improving your health and fitness has just been
made easier and more convenient with our Active
Bassetlaw Card.
Our Direct Debit scheme offers unrivalled
opportunities to stay fit and healthy in the
District. Membership packages
start at £19.50 a month
(that’s just 70p a day!) and
allow customers use of
the swim, gym and fitness
classes at Bircotes, Retford and
Worksop Leisure Centre.
Five different membership options
are available including Junior
Memberhips and an off peak option,
allowing use of the gym until 5.00pm weekdays
and all day at the weekends.

An initial joining fee of £20.00 is applicable to all categories of membership
although it is possible to pay by lump sum and the joining fee will then be wavered.
Direct Debit Paying by direct debit is the easiest and most convenient
way of accessing the facilities that Bassetlaw now offer, giving you the
opportunity of a more active and healthier lifestyle.
What Activities Are Available There are plenty of activities to chose
from at all 3 Leisure centres whether its in the pool or our
comprehensive dry-side programme with over 50 Hours a week
of Public swimming and over 100 Hours a week Gym/Fitness
activities.
The Leisure Card Scheme
Aims to bring Bassetlaw’s outstanding facilities to everyone at the
lowest possible cost. The Card is valid for one year from the dat e
of issue and can be used at any of the Councils Leisure Centres at
Bircotes, Retford and Worksop. The card is also valid at Kilton
Forest Golf Course, and many of the outdoor recreation areas
including:

• Kings Park • Canch • Costhorpe Memorial Grounds

The application forms are available at any of the Bassetlaw Leisure Centres and the
Kilton Forest Golf Course. The forms will also be available to download on-line at

Part 1 Personal Details These details will be treated in confidence and used to ensure that the activities and
programmes provided will be safe and suited specifically for your individual needs. Please fill in all the details
in full although some questions are optional and marked with an asterix. On the medical conditions part
of the form please fill in any relevant medication you may be taking.
Part 2 Direct Debit This covers your Health and Fitness membership. The memberships can be
taken out for a minimum of 6 or 12 months. A £20 joining fee will be applicable for your firs
tmembership. Choosing the Direct Debit options is the easiest and most convenient way of
making the most of the new facilities leading you to a more active and healthier lifestyle.
Part 3 Loyalty Card Details The Card can be used at any of the Councils new Leisure
Centres, the outdoor amenity sites and also at Kilton Forest Golf Course. The cost of the
Card and the reduction in the cost of the activities will depend on whether you live inor
out of Bassetlaw and whether or not you qualify for a concession. If you are in any
doubt please contact the receptions at any of the Leisure Centre’s.
When you have completed the form please take them to your nearest Leisure Centre
and have them checked. You will need to have the following:
1. The completed application form.
2. Proof of any concessions you are applying for
3. Payment/Completed Direct Mandate Form
The documents will be checked for their validity and a photo taken on site, which will
beused for verification. The Active Card will then be issued on site and you will be able
to use it straight away. Any information stored will be kept under the Data protection Act
1998 and used only to improve the delivery of the service.
