the school district provides the parent with a written proposal to amend a<br />

provision or provisions of the IEP that:<br />

o is conveyed in language understandable to the parent, and<br />

o informs and allows the parents the opportunity to consult with the<br />

appropriate personnel or related service providers concerning the<br />

proposed changes.<br />

The parent must agree in writing to such amendments.<br />

The parent must be provided prior written notice (notice of the<br />

recommendation) of the changes to the IEP.<br />

The CPSE must be notified of any changes made to the IEP.<br />

The parent must also receive, either a<br />

o rewritten IEP; or<br />

o document that amends or modifies the IEP, or upon parental<br />

request, a revised copy of the entire IEP with amendments<br />

incorporated.<br />

Amendments made to an IEP without a meeting do not affect the requirement<br />

that the CPSE must review the IEP at the annual meeting, or more often if<br />

necessary.<br />

Withdrawal of Referral<br />

Written consent of the parent or guardian is required prior to initiating an<br />

evaluation for a student who has not previously been identified as having a<br />

disability. In the event that parent permission is withheld, the parent shall be<br />

given the opportunity to attend an informal conference with a designated<br />

professional most familiar with the proposed evaluation. If, at this meeting the<br />

parent decides that the referral is not warranted, the referral shall be withdrawn.<br />
