1.4 Making an application under the Act<br />

In compiling your application, please note the following guidelines:<br />

• Applications should normally be in writing. You may use the form<br />

entitled ‘Request for Information under the Freedom <strong>of</strong> Information<br />

Act,’ (Appendix D) which can be downloaded from the <strong>Institute</strong><br />

website (www.itb.ie ) or can be obtained by contacting the <strong>of</strong>fice<br />

below.<br />

• If you are not using the form outlined in Appendix D, your<br />

application should indicate that the information is sought under FOI<br />

(1997).<br />

• If possible, include a day-time telephone number, so that you may<br />

be contacted quickly if it is necessary to clarify details <strong>of</strong> your<br />

request.<br />

• Indicate if the information is required in a particular format (e.g.,<br />

photocopy, computer disc).<br />

• Include as much detail as possible to assist the staff <strong>of</strong> the <strong>Institute</strong><br />

to identify the record. Indicate the time period for which you wish<br />

to access records (e.g., records created between May 1998 and<br />

December 1998). Specify the areas <strong>of</strong> the <strong>Institute</strong> which are most<br />

relevant to your request. If you have difficulty in identifying the<br />

precise records which you require, the staff <strong>of</strong> the FOI unit will be<br />

happy to assist you in preparing your requests.<br />

• Requests for information may also be made by fax, by e-mail, or by<br />

writing to the <strong>of</strong>fice address below. You may be required to prove<br />

your identity, especially when requesting personal information. You<br />

may, therefore, be asked to produce your passport, driving license<br />

or birth certificate.<br />
