Introduction
The Culinary Health Fund is a multi-employer Taft-Hartley Health and Welfare Trust
governed by a Board of Trustees representing both labor and management. It is
funded by employer contributions negotiated in Collective Bargaining Agreements.
Contracts for Culinary Health Fund plan providers are maintained through the Culinary
Health Fund Administrative Services, LLC.
The Culinary Health Fund is one of the largest health plans in Nevada, covering
approximately 120,000 lives.
The Culinary Health Fund has developed this guide in order for providers and their office
staff to navigate through the Culinary Health Fund plan simply and easily.
This guide provides helpful information regarding claims submission and payment, prior
authorization, and benefits.
Information in this guide will be updated regularly. Updates are available via facsimile and/
or mail to your office and can also be found on the Culinary Health Fund’s website at
