• <strong>Nurses</strong> are lead<strong>in</strong>g <strong>in</strong> <strong>the</strong> delivery <strong>of</strong> quality patient<br />

care <strong>in</strong> a transformed health care system.<br />

• Both <strong>the</strong> Affordable Care Act (ACA) and <strong>the</strong><br />

Institute <strong>of</strong> Medic<strong>in</strong>e’s Future <strong>of</strong> Nurs<strong>in</strong>g<br />

report have called for our pr<strong>of</strong>essi<strong>on</strong> to optimize<br />

its c<strong>on</strong>tributi<strong>on</strong> to better meet <strong>the</strong> needs <strong>of</strong> all<br />

patients for quality health care.<br />

• <strong>Nurses</strong> must seize <strong>the</strong> opportunity to dem<strong>on</strong>strate<br />

nurs<strong>in</strong>g’s value <strong>in</strong> ec<strong>on</strong>omic terms.<br />

• Over <strong>the</strong> years, nurs<strong>in</strong>g has d<strong>on</strong>e a good job<br />

<strong>of</strong> dem<strong>on</strong>strat<strong>in</strong>g its social value, <strong>in</strong>clud<strong>in</strong>g its<br />

commitment to patient care, improved safety and<br />

quality, and better outcomes.<br />

• But with <strong>the</strong> <strong>in</strong>creas<strong>in</strong>g drumbeat to c<strong>on</strong>trol<br />

costs <strong>in</strong> health care, it’s become critical that we<br />

also dem<strong>on</strong>strate nurs<strong>in</strong>g’s ec<strong>on</strong>omic value – and<br />

<strong>the</strong> return <strong>on</strong> <strong>in</strong>vestment it br<strong>in</strong>gs.<br />

• Def<strong>in</strong><strong>in</strong>g and articulat<strong>in</strong>g <strong>the</strong> ec<strong>on</strong>omic value<br />

we br<strong>in</strong>g to quality care will help ensure policymakers,<br />

hospital executives and o<strong>the</strong>rs make<br />

<strong>in</strong>formed decisi<strong>on</strong>s about quality care and <strong>the</strong><br />

essential role <strong>of</strong> nurs<strong>in</strong>g today and <strong>in</strong> <strong>the</strong> future.<br />

• <strong>Nurses</strong> impact <strong>the</strong> bottom l<strong>in</strong>e by hold<strong>in</strong>g down<br />

costs.<br />

• We know that as nurse staff<strong>in</strong>g levels <strong>in</strong>crease,<br />

patient risk <strong>of</strong> complicati<strong>on</strong>s and hospital lengths<br />

<strong>of</strong> stay decrease, result<strong>in</strong>g <strong>in</strong> cost sav<strong>in</strong>gs.<br />

• We also know that nurse-managed care<br />

coord<strong>in</strong>ati<strong>on</strong> and transiti<strong>on</strong>al care decrease costs<br />

by reduc<strong>in</strong>g emergency department<br />

visits and hospital readmissi<strong>on</strong>s and lower<strong>in</strong>g<br />

• And based <strong>on</strong> what we know today, we can<br />

• But nurses must ensure that key decisi<strong>on</strong> makers<br />

know this too.<br />

• Step <strong>on</strong>e is to stay <strong>in</strong>formed <strong>on</strong> health care policy<br />

and health care f<strong>in</strong>anc<strong>in</strong>g issues that drive<br />

decisi<strong>on</strong>s relat<strong>in</strong>g to nurs<strong>in</strong>g practice.<br />

• Step two is to participate <strong>in</strong> workforce plann<strong>in</strong>g<br />

and data collecti<strong>on</strong> opportunities with<strong>in</strong> your<br />

practice sett<strong>in</strong>g. As nurses, we must measure<br />

<strong>the</strong> value <strong>of</strong> what we do – and that is more<br />

important today than ever before.<br />

• Step three is to advocate <strong>on</strong> behalf <strong>of</strong> nurses at<br />

all levels <strong>of</strong> <strong>the</strong> health care system as we<br />

implement new policies and provide patients with<br />

quality and affordable care.<br />

• Most important, engage with your pr<strong>of</strong>essi<strong>on</strong>al<br />

associati<strong>on</strong> to amplify your voice.<br />

• Your pr<strong>of</strong>essi<strong>on</strong>al associati<strong>on</strong> is your partner <strong>in</strong><br />

maximiz<strong>in</strong>g this opportunity.<br />

• Pr<strong>of</strong>essi<strong>on</strong>al associati<strong>on</strong>s leverage <strong>the</strong> collective<br />

energy and resources <strong>of</strong> many nurses to <strong>in</strong>fluence<br />

• Reimbursement through <strong>the</strong> Centers for Medicare<br />

& Medicaid Services (CMS) represents 45 percent<br />

<strong>of</strong> hospital revenues. (CMS, NHA 2011)<br />

• In 2013, <strong>the</strong> first year <strong>in</strong> which payments will<br />

be made under Medicare’s <strong>Value</strong> Based<br />

Purchas<strong>in</strong>g (VBP) program, patient satisfacti<strong>on</strong><br />

A new era for nurses has<br />

begun <strong>in</strong> South Carol<strong>in</strong>a.<br />

A recent state-level Nurs<strong>in</strong>g<br />

Advisory Committee’s discussi<strong>on</strong><br />

revealed many new nurses,<br />

especially ADN’s, are not<br />

be<strong>in</strong>g able to f<strong>in</strong>d jobs after<br />

graduati<strong>on</strong>. Three hospitals <strong>in</strong><br />

SC are hir<strong>in</strong>g <strong>on</strong>ly BSN’s. Many<br />

o<strong>the</strong>rs <strong>on</strong>ly want experienced<br />

nurses. <strong>The</strong> hospital associati<strong>on</strong><br />

has some <strong>in</strong>formati<strong>on</strong> <strong>on</strong> <strong>the</strong><br />

turn <strong>of</strong> events and is <strong>in</strong> <strong>the</strong> Vicki C. Green<br />

process to ga<strong>the</strong>r even more<br />

supportive data.<br />

On a pers<strong>on</strong>al note, my positi<strong>on</strong> with a state agency was<br />

just elim<strong>in</strong>ated – al<strong>on</strong>g with 45 o<strong>the</strong>rs, many <strong>of</strong> whom are<br />

nurs<strong>in</strong>g leaders. Some were fortunate and were hired back<br />

<strong>in</strong>to <strong>on</strong>e <strong>of</strong> <strong>the</strong> 63 newly-created positi<strong>on</strong>s. O<strong>the</strong>rs elected<br />

to retire. Some took o<strong>the</strong>r vacant positi<strong>on</strong>s or left <strong>the</strong><br />

agency. As it has all unfolded, say<strong>in</strong>g goodbye to 28-yearsworth<br />

<strong>of</strong> friends and colleagues who worked to improve<br />

<strong>the</strong> health <strong>of</strong> <strong>the</strong> public is very sad.<br />

<strong>The</strong>se events have proven <strong>the</strong>re is little job security left<br />

<strong>the</strong>se days. As my nurs<strong>in</strong>g colleagues <strong>in</strong> bus<strong>in</strong>ess have l<strong>on</strong>g<br />

known for decades, new leadership can enter, decide <strong>the</strong>y<br />

need new people for whatever reas<strong>on</strong>/whim and elim<strong>in</strong>ate<br />

positi<strong>on</strong>s. My advice, for whatever it’s worth, is to stay<br />

focused <strong>on</strong> your job and do it to <strong>the</strong> best <strong>of</strong> your ability.<br />

<strong>The</strong>n, when you walk away, you can hold your head high<br />

– know<strong>in</strong>g you did your best and <strong>the</strong>y are gett<strong>in</strong>g rid <strong>of</strong> <strong>the</strong><br />

best!<br />

As much as I would love to reveal all <strong>of</strong> <strong>the</strong> details<br />

<strong>of</strong> <strong>the</strong> events that have lead up to my departure from<br />

<strong>the</strong> agency, I choose to keep those who are guilty <strong>of</strong> illtreatment<br />

an<strong>on</strong>ymous. <strong>The</strong>y have to live with <strong>the</strong>ir acti<strong>on</strong>s<br />

and <strong>the</strong>ir c<strong>on</strong>sciences. As for <strong>the</strong> benefit <strong>of</strong> those <strong>of</strong> us who<br />

were harmed, <strong>the</strong>re will be a day <strong>of</strong> reck<strong>on</strong><strong>in</strong>g!<br />

Hav<strong>in</strong>g my positi<strong>on</strong> elim<strong>in</strong>ated and not be<strong>in</strong>g hired<br />

back to c<strong>on</strong>t<strong>in</strong>ue my beloved career was ak<strong>in</strong> to be<strong>in</strong>g<br />

tripped <strong>on</strong> purpose, <strong>the</strong>n be<strong>in</strong>g kicked <strong>in</strong> <strong>the</strong> gut while<br />

still <strong>on</strong> <strong>the</strong> ground. Besides many sleepless nights, it has<br />

caused me to become very reflective. Like <strong>the</strong> adage, “you<br />

can’t c<strong>on</strong>trol what happens to you, but you can c<strong>on</strong>trol<br />

how you react to it”, I am <strong>in</strong> <strong>the</strong> process <strong>of</strong> gett<strong>in</strong>g myself<br />

up <strong>of</strong>f <strong>the</strong> floor and deal<strong>in</strong>g with it. Luckily, I had enough<br />

years <strong>in</strong> <strong>the</strong> system to retire and walk away. For that, I am<br />

very grateful.<br />

I am also very grateful for <strong>the</strong> opportunities I had<br />

with<strong>in</strong> public health and my nurs<strong>in</strong>g career. I had<br />

w<strong>on</strong>derful mentors – like Dr. Lucy Mari<strong>on</strong>, who hired me<br />

as an <strong>of</strong>fice nurse when she worked <strong>in</strong> a private practice as<br />

an APRN. She not <strong>on</strong>ly c<strong>on</strong>v<strong>in</strong>ced me I needed to go back<br />

to school, she signed up for a Biostatistics class with me!<br />

Ano<strong>the</strong>r mentor is Deloris Zeigler. Deloris lured me<br />

<strong>in</strong>to public health – not <strong>on</strong>ce, but twice – and showed<br />

me what it took to ma<strong>in</strong>ta<strong>in</strong> <strong>in</strong>tegrity while work<strong>in</strong>g <strong>in</strong><br />

a very political envir<strong>on</strong>ment. Deloris made sure I had<br />

opportunities to grow as a leader. Am<strong>on</strong>g <strong>the</strong>m, she<br />

nom<strong>in</strong>ated me for president <strong>of</strong> <strong>the</strong> local SCNA chapter –<br />

and <strong>the</strong> SC <strong>Nurses</strong> Foundati<strong>on</strong>, where I served for 9 years,<br />

2 <strong>of</strong> which were as president.<br />

Ann Lee mentored me as a nurs<strong>in</strong>g director and<br />

provided many opportunities to grow as a leader – formal<br />

and <strong>in</strong>formal. As a “baby” nurs<strong>in</strong>g director, she nom<strong>in</strong>ated<br />

me for <strong>the</strong> Amy Cockcr<strong>of</strong>t Fellowship at USC where I<br />

learned a lot about myself and my leadership style. Later<br />

<strong>in</strong> my career, she pushed me to run for president <strong>of</strong> SCNA.<br />

AND… I’m still here!<br />

I am <strong>in</strong>debted to all <strong>of</strong> my nurs<strong>in</strong>g mentors, fellow<br />

nurs<strong>in</strong>g directors, and my w<strong>on</strong>derful staff who supported<br />

me over <strong>the</strong> years. However, my most recent mentor is not<br />

a nurse. Judy Thomps<strong>on</strong>, our own CEO, is an outstand<strong>in</strong>g<br />

mentor. She is always <strong>the</strong>re with good advice, but allows<br />

will be a little early, but, Happy<br />

<strong>Nurses</strong> Week, 2013! <strong>The</strong><br />

<strong>the</strong>me this year is “Deliver<strong>in</strong>g<br />

Quality and Innovati<strong>on</strong> <strong>in</strong><br />

Patient Care.” This is a great<br />

<strong>the</strong>me and a c<strong>on</strong>t<strong>in</strong>uati<strong>on</strong><br />

<strong>of</strong> <strong>the</strong> work d<strong>on</strong>e s<strong>in</strong>ce <strong>the</strong><br />

beg<strong>in</strong>n<strong>in</strong>g <strong>of</strong> organized nurs<strong>in</strong>g.<br />

<strong>Nurses</strong> have always used <strong>the</strong>ir<br />

educati<strong>on</strong>s and skills to work<br />

for <strong>the</strong> improvement <strong>of</strong> care<br />

and to deliver <strong>the</strong> highest<br />

quality <strong>of</strong> patient care possible.<br />

<strong>in</strong>novative patient care.<br />

Perhaps <strong>on</strong>e <strong>of</strong> <strong>the</strong> best ways to celebrate <strong>Nurses</strong> Week,<br />

May 6-12, 2013, is to revisit some <strong>of</strong> <strong>the</strong> important impact<br />

<strong>the</strong> pr<strong>of</strong>essi<strong>on</strong> <strong>of</strong> nurs<strong>in</strong>g has <strong>on</strong> <strong>the</strong> health care system <strong>of</strong><br />

today. <strong>Nurses</strong> are lead<strong>in</strong>g <strong>in</strong> <strong>the</strong> delivery <strong>of</strong> quality patient<br />

care <strong>in</strong> a system <strong>of</strong> health care delivery that is rapidly<br />

be<strong>in</strong>g transformed. As part <strong>of</strong> this transformati<strong>on</strong> it is <strong>of</strong><br />

vital importance that nurs<strong>in</strong>g’s value to <strong>the</strong> health care<br />

system is seen not <strong>on</strong>ly <strong>in</strong> <strong>the</strong> excellence <strong>of</strong> care giv<strong>in</strong>g,<br />

but, <strong>in</strong> <strong>the</strong> terms <strong>of</strong> <strong>the</strong> ec<strong>on</strong>omic value that nurs<strong>in</strong>g br<strong>in</strong>gs<br />

to health care.<br />

It is well known that with <strong>in</strong>creases <strong>in</strong> nurs<strong>in</strong>g staff<br />

levels <strong>the</strong> risks for patient complicati<strong>on</strong>s and hospital<br />

lengths <strong>of</strong> stay decrease, thus result<strong>in</strong>g <strong>in</strong> cost sav<strong>in</strong>gs<br />

to <strong>in</strong>stituti<strong>on</strong>s. Nurse-managed care coord<strong>in</strong>ati<strong>on</strong> and<br />

transiti<strong>on</strong>al care also assist <strong>in</strong> decreas<strong>in</strong>g costs by<br />

reduc<strong>in</strong>g emergency department visits and hospital<br />

readmissi<strong>on</strong> thus lower<strong>in</strong>g Medicare costs.<br />

It is certa<strong>in</strong>ly time for nurses to be very clear about <strong>the</strong><br />

excellent value that each <strong>on</strong>e br<strong>in</strong>gs to <strong>the</strong> care <strong>of</strong> patients<br />

at all levels <strong>of</strong> car<strong>in</strong>g. <strong>The</strong> evidence <strong>of</strong> this value c<strong>on</strong>t<strong>in</strong>ues<br />

to pile up <strong>in</strong> study after study. <strong>Nurses</strong> are true valueadders<br />

<strong>in</strong> <strong>the</strong> health care system <strong>of</strong> today and will be even<br />

more important <strong>in</strong> <strong>the</strong> future.<br />

So, celebrate you! Celebrate <strong>the</strong> pr<strong>of</strong>essi<strong>on</strong> that you<br />

have chosen to be part <strong>of</strong>! Celebrate <strong>the</strong> important roles<br />

that you play <strong>in</strong> health care today! Celebrate <strong>the</strong> work<br />

that you do by “Deliver<strong>in</strong>g Quality and Innovati<strong>on</strong> <strong>in</strong><br />

Patient Care!” ONWARD!<br />

me to make my own decisi<strong>on</strong>s. She is so respected – such<br />

a str<strong>on</strong>g leader, both state and nati<strong>on</strong>ally. I look forward to<br />

a couple more years <strong>of</strong> benefit<strong>in</strong>g from her mentor<strong>in</strong>g.<br />

I am thankful for hav<strong>in</strong>g this <strong>of</strong>fice and this venue to<br />

communicate. It has afforded me many opportunities. <strong>The</strong><br />

latest opportunity was speak<strong>in</strong>g to <strong>the</strong> SCMA’s Bioethics<br />

Committee <strong>on</strong> <strong>the</strong> scope <strong>of</strong> practice for APRN’s. I am<br />

h<strong>on</strong>ored that <strong>the</strong>y have asked me to come back to <strong>the</strong>ir<br />

next meet<strong>in</strong>g and potentially jo<strong>in</strong><strong>in</strong>g <strong>the</strong> committee as a<br />

nurs<strong>in</strong>g advocate.<br />

Hopefully, <strong>in</strong> this new era for nurs<strong>in</strong>g, I have stayed<br />

focused <strong>on</strong> <strong>the</strong> positive. We nurses have chosen a very<br />

ethical, trustworthy pr<strong>of</strong>essi<strong>on</strong>. In all that we do <strong>in</strong> <strong>the</strong><br />

name <strong>of</strong> nurs<strong>in</strong>g, we must stay focused <strong>on</strong> <strong>the</strong> positive.<br />

We need to encourage those who are c<strong>on</strong>templat<strong>in</strong>g or<br />

just start<strong>in</strong>g <strong>the</strong>ir pr<strong>of</strong>essi<strong>on</strong>s. We must help <strong>the</strong>m see <strong>the</strong><br />

positives will far outweigh <strong>the</strong> negatives <strong>in</strong> <strong>the</strong> end.<br />

Advocacy Fund Created<br />

<strong>The</strong> SCNA Board <strong>of</strong> Directors has created <strong>the</strong> SCNA<br />

ADVOCACY FUND to assist <strong>in</strong> support<strong>in</strong>g <strong>the</strong> advocacy<br />

work already d<strong>on</strong>e by <strong>the</strong> SCNA <strong>on</strong> behalf <strong>of</strong> <strong>the</strong><br />

pr<strong>of</strong>essi<strong>on</strong> <strong>of</strong> nurs<strong>in</strong>g. This fund will assist <strong>in</strong> cover<strong>in</strong>g <strong>the</strong><br />

costs <strong>of</strong> <strong>the</strong> advocacy work d<strong>on</strong>e by SCNA. <strong>The</strong>se costs<br />

have been <strong>in</strong>creas<strong>in</strong>g as opportunities for acti<strong>on</strong> have also<br />

<strong>in</strong>creased. <strong>The</strong>se opportunities are <strong>in</strong> all areas <strong>of</strong> nurs<strong>in</strong>g<br />

practice. <strong>The</strong> Fund was unveiled dur<strong>in</strong>g <strong>the</strong> SCNA APRN<br />

Chapter’s workshop recently held <strong>in</strong> Spartanburg, South<br />

Carol<strong>in</strong>a. <strong>The</strong> attendees were most generous <strong>in</strong> gett<strong>in</strong>g <strong>the</strong><br />

Fund <strong>of</strong>f to a good beg<strong>in</strong>n<strong>in</strong>g.<br />

All members <strong>of</strong> <strong>the</strong> SCNA Board <strong>of</strong> Directors have also<br />

been asked to c<strong>on</strong>tribute <strong>in</strong> additi<strong>on</strong> to all <strong>the</strong> volunteer<br />

time that <strong>the</strong>y provide for advocacy <strong>on</strong> behalf <strong>of</strong> <strong>the</strong><br />

nurs<strong>in</strong>g pr<strong>of</strong>essi<strong>on</strong>. SCNA Chapter members will also be<br />

challenged to participate <strong>in</strong> this excit<strong>in</strong>g effort.<br />

<strong>The</strong> Advocacy Fund will augment <strong>the</strong> SCNA<br />

budgeted amounts that are needed for a variety <strong>of</strong> costs<br />

related to advocacy. C<strong>on</strong>tributi<strong>on</strong>s may be made by<br />

any<strong>on</strong>e to SCNA. <strong>The</strong> Fund is not a tax-deductible item<br />

for <strong>in</strong>dividuals due to <strong>the</strong> fact that it will be used for<br />

advocacy and lobby<strong>in</strong>g. D<strong>on</strong>ati<strong>on</strong>s may be made us<strong>in</strong>g <strong>the</strong><br />

Stoker <strong>of</strong> AnMed were named recently as announced by<br />

<strong>The</strong> first order <strong>of</strong> bus<strong>in</strong>ess for <strong>the</strong> SCNA members <strong>of</strong><br />

<strong>the</strong> Board was to attend <strong>the</strong> Annual meet<strong>in</strong>g <strong>of</strong> <strong>the</strong> South<br />

Carol<strong>in</strong>a <strong>Nurses</strong> Foundati<strong>on</strong>. <strong>The</strong> SCNA Board members<br />

serve as <strong>the</strong> members <strong>of</strong> <strong>the</strong> SCNF and with <strong>the</strong> Trustees<br />

<strong>of</strong> <strong>the</strong> SCNF. Follow<strong>in</strong>g this meet<strong>in</strong>g, <strong>the</strong> SCNA Board<br />

ga<strong>the</strong>red to c<strong>on</strong>duct <strong>the</strong> SCNA Board meet<strong>in</strong>g.<br />

<strong>The</strong> M<strong>in</strong>utes <strong>of</strong> a number <strong>of</strong> meet<strong>in</strong>gs were approved as<br />

circulated.<br />

<strong>The</strong> F<strong>in</strong>ance reports from October, November,<br />

December and January were accepted as circulated. <strong>The</strong><br />

adopti<strong>on</strong> <strong>of</strong> <strong>the</strong> 2013 budget was reaffirmed by <strong>the</strong> Board.<br />

<strong>The</strong> President-elect was appo<strong>in</strong>ted to serve as <strong>the</strong><br />

SCNA representative to <strong>the</strong> SCNF with vot<strong>in</strong>g power.<br />

Additi<strong>on</strong>s to <strong>the</strong> Nom<strong>in</strong>ati<strong>on</strong>s Committee were made to<br />

support <strong>the</strong> work <strong>of</strong> Chair Lois Hasan. <strong>The</strong> three current<br />

Directors were added to <strong>the</strong> committee.<br />

Alice Wyatt volunteered to work with <strong>the</strong> CEO to<br />

review <strong>the</strong> current SCNA Bylaws for any proposed<br />

changes and to review any changes proposed by <strong>the</strong><br />

members for <strong>the</strong> SCNA Annual Meet<strong>in</strong>g <strong>in</strong> October.<br />

<strong>The</strong> SCNA Chapters were <strong>on</strong>ce aga<strong>in</strong> will<strong>in</strong>g to<br />

participate <strong>in</strong> creat<strong>in</strong>g awards from each Chapter to be<br />

presented at <strong>the</strong> SCNA C<strong>on</strong>venti<strong>on</strong> <strong>in</strong> October. This<br />

is <strong>the</strong> same pattern that was used at <strong>the</strong> last C<strong>on</strong>venti<strong>on</strong><br />

and it was very successful. <strong>The</strong> awards <strong>the</strong>mselves are<br />

certificates and <strong>the</strong> creati<strong>on</strong> <strong>of</strong> a brick for <strong>the</strong> SCNA<br />

walkway.<br />

Written Reports were received from all <strong>the</strong> Officers,<br />

Commissi<strong>on</strong> Chairs, Directors and staff as requested and<br />

greatly facilitated <strong>the</strong> flow <strong>of</strong> <strong>the</strong> meet<strong>in</strong>g.<br />

A special committee to review and update <strong>the</strong> SCNA<br />

Strategic Plan was created with volunteers: Peggy<br />

Dulaney, C<strong>on</strong>nie Varn, Lawrence Eberl<strong>in</strong>, Vicki Green,<br />

Judith Thomps<strong>on</strong> and Rosie Rob<strong>in</strong>s<strong>on</strong>.<br />

Nurse Practiti<strong>on</strong>ers will meet at <strong>the</strong> SC State House <strong>on</strong><br />

April 18th for a time to talk with members <strong>of</strong> <strong>the</strong> General<br />

Board <strong>of</strong> Directors Update<br />

Acti<strong>on</strong> <strong>of</strong> <strong>the</strong> February 22, 2013 Meet<strong>in</strong>g <strong>of</strong> <strong>the</strong><br />

SCNA Board <strong>of</strong> Directors<br />

Assembly about <strong>the</strong>ir practices. O<strong>the</strong>r legislative issues are<br />

mov<strong>in</strong>g forward as well at this time.<br />

A new policy for <strong>the</strong> CEAC was approved to replace<br />

<strong>the</strong> former policy <strong>of</strong> pay<strong>in</strong>g mileage for members <strong>of</strong> <strong>the</strong><br />

committee. This new policy will provide compensati<strong>on</strong> to<br />

all members <strong>of</strong> <strong>the</strong> CEAC not just those who live outside<br />

<strong>of</strong> Columbia and will be based <strong>on</strong> a fee per quarter for<br />

each committee member who has participated <strong>in</strong> <strong>the</strong> full<br />

work <strong>of</strong> <strong>the</strong> committee for <strong>the</strong> quarter. <strong>The</strong> Chair’s fee<br />

will be $200.00 per quarter and each committee member<br />

will receive $100.00.<br />

<strong>The</strong> annual orientati<strong>on</strong> for members <strong>of</strong> <strong>the</strong> Board<br />

<strong>of</strong> Directors and Chapter Chairs took place us<strong>in</strong>g <strong>the</strong><br />

updated Positi<strong>on</strong> Descripti<strong>on</strong>s found <strong>in</strong> <strong>the</strong> SCNA Boar<br />

d <strong>of</strong> Directors and Committee Manual. CEO Thomps<strong>on</strong><br />

reviewed all <strong>the</strong> Positi<strong>on</strong> Descripti<strong>on</strong>s and Policies <strong>of</strong><br />

SCNA and made proposed changes based <strong>on</strong> Bylaw<br />

changes and o<strong>the</strong>r factors impact<strong>in</strong>g <strong>the</strong> work <strong>of</strong> <strong>the</strong> Board<br />

and Committees. This proved to be a good way to perform<br />

<strong>the</strong> orientati<strong>on</strong> for this year as it touched <strong>on</strong> every aspect<br />

<strong>of</strong> SCNA <strong>in</strong> a brief manner and allowed for <strong>in</strong>put from<br />

<strong>the</strong> Board members. <strong>The</strong>re are three topic areas still to<br />

be specifically reviewed by <strong>the</strong> chairs <strong>of</strong> <strong>the</strong> Committees<br />

<strong>in</strong>volved and will be presented for <strong>the</strong> next Board meet<strong>in</strong>g.<br />

<strong>The</strong> next Board meet<strong>in</strong>g is May 17, 2013.<br />

How w<strong>on</strong>derful to see all <strong>the</strong> many directi<strong>on</strong>s that <strong>the</strong><br />

field <strong>of</strong> nurs<strong>in</strong>g is go<strong>in</strong>g <strong>in</strong> <strong>the</strong>se days. I am so proud and<br />

happy when I sit back and c<strong>on</strong>template <strong>the</strong> course that I<br />

have seen this amaz<strong>in</strong>g pr<strong>of</strong>essi<strong>on</strong> take over <strong>the</strong> last forty<br />

plus years. I have had <strong>the</strong> privilege <strong>of</strong> be<strong>in</strong>g <strong>in</strong> <strong>the</strong> field <strong>of</strong><br />

psychiatric nurs<strong>in</strong>g s<strong>in</strong>ce 1967. I started out as a nurs<strong>in</strong>g<br />

assistant while I earned my associate degree <strong>in</strong> nurs<strong>in</strong>g<br />

here at <strong>the</strong> University <strong>of</strong> South Carol<strong>in</strong>a.<br />

Slowly over <strong>the</strong> years as I worked <strong>in</strong>-patient psychiatric<br />

nurs<strong>in</strong>g and <strong>the</strong>n outpatient, I f<strong>in</strong>ished my BS <strong>in</strong> Nurs<strong>in</strong>g<br />

and <strong>the</strong>n eventually my Masters! What a joy it has been to<br />

work as a psychiatric nurse for so many years now. Over<br />

<strong>the</strong> years <strong>in</strong> my private psycho<strong>the</strong>rapy practice here <strong>in</strong><br />

Columbia, I have noticed what an enormous role stress has<br />

played <strong>in</strong> lives <strong>of</strong> my clients. I might say that I have also<br />

been acutely aware <strong>of</strong> <strong>the</strong> role <strong>of</strong> stress <strong>in</strong> my own life, and<br />

<strong>the</strong> life <strong>of</strong> my family and my friends.<br />

About seven years ago I had a series <strong>of</strong> very difficult<br />

events <strong>in</strong> my life that took my stress level bey<strong>on</strong>d anyth<strong>in</strong>g<br />

I had ever had to deal with. In additi<strong>on</strong> to <strong>the</strong> many<br />

skills that I had already learned for stress management, I<br />

decided to add a yoga and meditati<strong>on</strong> practice to my list<br />

<strong>of</strong> tools. To this day, I still marvel at what a significant<br />

difference <strong>the</strong>se tools have made <strong>in</strong> my life.<br />

Because <strong>the</strong>se “m<strong>in</strong>dfulness” practices helped me so<br />

much <strong>in</strong> my pers<strong>on</strong>al life, I began to study this area <strong>of</strong><br />

m<strong>in</strong>d-body medic<strong>in</strong>e more extensively. I wanted to br<strong>in</strong>g<br />

to my patients <strong>the</strong> benefits <strong>of</strong> <strong>the</strong>se practices.<br />

One <strong>of</strong> <strong>the</strong> most user friendly methods that I have<br />

discovered al<strong>on</strong>g <strong>the</strong> way is called M<strong>in</strong>dfulness Based<br />

Stress Reducti<strong>on</strong> (MBSR). In 1979 J<strong>on</strong> Kabat-Z<strong>in</strong>n<br />

founded MBSR <strong>in</strong> his stress-reducti<strong>on</strong> cl<strong>in</strong>ic at UMass<br />

Medical Center <strong>in</strong> Worcester. S<strong>in</strong>ce that time programs<br />

have sprung up all around <strong>the</strong> world with many variati<strong>on</strong>s<br />

such as M<strong>in</strong>dfulness Based Stress Reducti<strong>on</strong> for Chr<strong>on</strong>ic<br />

<strong>in</strong> nurs<strong>in</strong>g educati<strong>on</strong>. Cas<strong>on</strong> has years <strong>of</strong> experience as a<br />

critical care nurse, a hospice nurse, hospital supervisor,<br />

and nurs<strong>in</strong>g <strong>in</strong>structor. She has been a recipient <strong>of</strong> <strong>the</strong><br />

South Carol<strong>in</strong>a <strong>Nurses</strong> Foundati<strong>on</strong> Palmetto Gold Award<br />

for Nurs<strong>in</strong>g Excellence as well as <strong>the</strong> South Carol<strong>in</strong>a<br />

League for Nurs<strong>in</strong>g Award for Excellence.<br />

To expand <strong>the</strong> science <strong>of</strong> nurs<strong>in</strong>g educati<strong>on</strong> while<br />

develop<strong>in</strong>g <strong>the</strong>ir pers<strong>on</strong>al leadership portfolios,<br />

participants spend time engaged <strong>in</strong> varied activities that<br />

exam<strong>in</strong>e key issues related to simulati<strong>on</strong>, <strong>the</strong>n focus <strong>the</strong>ir<br />

efforts <strong>in</strong> an area <strong>of</strong> <strong>the</strong>ir choice. To kick <strong>of</strong>f <strong>the</strong> program,<br />

participants meet <strong>in</strong> January <strong>in</strong> Phoenix and attend <strong>the</strong><br />

NLN Leadership C<strong>on</strong>ference. Look<strong>in</strong>g ahead, <strong>the</strong> group<br />

will participate <strong>in</strong> leadership development web<strong>in</strong>ars;<br />

exchange ideas and best practices <strong>in</strong> simulati<strong>on</strong> <strong>in</strong> private<br />

forums; review exist<strong>in</strong>g scholarly research; visit simulati<strong>on</strong><br />

centers around <strong>the</strong> country to evaluate resources and<br />

operati<strong>on</strong>s; c<strong>on</strong>sult with Laerdal representatives <strong>on</strong><br />

equipment issues; c<strong>on</strong>tribute to a group project and post<br />

it <strong>on</strong> <strong>the</strong> Simulati<strong>on</strong> Innovati<strong>on</strong> Resource Center (SIRC)<br />

website; and attend c<strong>on</strong>ferences.<br />

In June <strong>of</strong> 2012 my colleague, Dr. Hilda White and I<br />

founded UPSTREAM Of Columbia: A Center for<br />

M<strong>in</strong>dfulness Practice and Holistic Mental Health.<br />

At this time we are <strong>of</strong>fer<strong>in</strong>g M<strong>in</strong>dfulness Based Stress<br />

Reducti<strong>on</strong> classes <strong>in</strong> eight week cycles throughout <strong>the</strong> year.<br />

<strong>The</strong> series c<strong>on</strong>sist <strong>of</strong> a two hour class eight weeks<br />

c<strong>on</strong>secutively and a <strong>on</strong>e day retreat held about two weeks<br />

before <strong>the</strong> series ends. <strong>The</strong>se psycho-educati<strong>on</strong>al classes,<br />

through a number <strong>of</strong> m<strong>in</strong>dfulness practices, teach students<br />

to become more aware <strong>of</strong> present moment experiencebe<strong>in</strong>g<br />

<strong>in</strong> <strong>the</strong> here and now without emoti<strong>on</strong>al reactivity or<br />

a story l<strong>in</strong>e about what is happen<strong>in</strong>g.<br />

Our m<strong>in</strong>ds are fantastic at project<strong>in</strong>g forward and<br />

th<strong>in</strong>k<strong>in</strong>g about all <strong>the</strong> future th<strong>in</strong>gs that we need to plan<br />

or to worry about! In additi<strong>on</strong> our m<strong>in</strong>ds are easily drawn<br />

<strong>in</strong>to <strong>the</strong> past rum<strong>in</strong>at<strong>in</strong>g <strong>on</strong> experiences from l<strong>on</strong>g ago.<br />

If we learn how to keep <strong>the</strong> m<strong>in</strong>d <strong>in</strong> <strong>the</strong> present, <strong>in</strong> <strong>the</strong><br />

here and now, us<strong>in</strong>g techniques like m<strong>in</strong>dfulness tra<strong>in</strong><strong>in</strong>g,<br />

<strong>the</strong> effects can be pr<strong>of</strong>ound. For example <strong>the</strong> effects may<br />

be <strong>in</strong> <strong>the</strong> physical body by lower<strong>in</strong>g blood pressure or<br />

reduc<strong>in</strong>g chr<strong>on</strong>ic pa<strong>in</strong> or <strong>the</strong>re may be mental health<br />

effects such as lower<strong>in</strong>g anxiety.<br />

As a nurse I am so delighted to be a part <strong>of</strong> <strong>of</strong>fer<strong>in</strong>g<br />

<strong>the</strong>se stress reducti<strong>on</strong> classes <strong>in</strong>to <strong>the</strong> Columbia area. As<br />

nurses we can all benefit from learn<strong>in</strong>g <strong>the</strong>se practices<br />

for ourselves and for our patients. At UPSTREAM we are<br />

proud to <strong>in</strong>troduce this program as part <strong>of</strong> <strong>the</strong> m<strong>in</strong>d-body<br />

medic<strong>in</strong>e field that is com<strong>in</strong>g <strong>in</strong>to its own at this time.<br />

Did you know that <strong>the</strong>re is a newly formed associati<strong>on</strong> for Faith Community <strong>Nurses</strong>?<br />

This associati<strong>on</strong> is an outgrowth <strong>of</strong> what was <strong>on</strong>ce a network <strong>of</strong> Faith Community <strong>Nurses</strong><br />

(Parish <strong>Nurses</strong>) who met toge<strong>the</strong>r for educati<strong>on</strong>, collaborati<strong>on</strong>, encouragement and to<br />

share best practices. As <strong>the</strong> specialty practice <strong>of</strong> Faith Community <strong>Nurses</strong> has grown<br />

and as <strong>the</strong> American <strong>Nurses</strong> Associati<strong>on</strong> <strong>in</strong> cooperati<strong>on</strong> with <strong>the</strong> Health M<strong>in</strong>istries<br />

Associati<strong>on</strong> jo<strong>in</strong>ed forces <strong>in</strong> develop<strong>in</strong>g unique standards for practice, it was thought<br />

prudent by <strong>the</strong> members <strong>of</strong> <strong>the</strong> network to form a legitimate associati<strong>on</strong> for members.<br />

In <strong>the</strong> past two years we have registered with <strong>the</strong> SC Secretary <strong>of</strong> State, applied for<br />

501-c-3 tax-exempt status, developed new by-laws, created a website (www.scfcna.<br />

com), became an Affiliate Member <strong>of</strong> <strong>the</strong> SCNA, and c<strong>on</strong>t<strong>in</strong>ued <strong>the</strong> good work that <strong>the</strong><br />

Network began.<br />

In order to practice as a Faith Community Nurse, <strong>the</strong> Standards for Faith Community<br />

<strong>Nurses</strong> recommend completi<strong>on</strong> <strong>of</strong> a Foundati<strong>on</strong>s Course. <strong>The</strong> SCFCNA will sp<strong>on</strong>sor a<br />

Foundati<strong>on</strong>s Course this spr<strong>in</strong>g <strong>in</strong> cooperati<strong>on</strong> with <strong>the</strong> Pee Dee AHEC.<br />

<strong>The</strong> course beg<strong>in</strong>s April 13, 2013 with a <strong>on</strong>e day sessi<strong>on</strong> to be held <strong>in</strong> Columbia<br />

and c<strong>on</strong>t<strong>in</strong>ues with a 3 day sessi<strong>on</strong> at Spr<strong>in</strong>gmaid Beach, SC from May 1–3, 2013.<br />

Completi<strong>on</strong> <strong>of</strong> <strong>the</strong> course will result <strong>in</strong> 34 c<strong>on</strong>tact hours <strong>of</strong> c<strong>on</strong>t<strong>in</strong>u<strong>in</strong>g educati<strong>on</strong>.<br />

<strong>the</strong> psych/mental health chapter<br />

wishes you all well. <strong>The</strong> chapter<br />

also wants to thank you for <strong>the</strong><br />

member <strong>in</strong>put that was received<br />

recently relat<strong>in</strong>g to meet<strong>in</strong>g <strong>the</strong><br />

members needs. <strong>The</strong> survey<br />

revealed that <strong>the</strong> desire <strong>of</strong> <strong>the</strong><br />

resp<strong>on</strong>dents was for <strong>the</strong> chapter<br />

to provide yearly c<strong>on</strong>t<strong>in</strong>u<strong>in</strong>g<br />

educati<strong>on</strong> opportunities. It was<br />

also suggested that we try to David Hods<strong>on</strong><br />

enhance network<strong>in</strong>g through<br />

our CEU <strong>of</strong>fer<strong>in</strong>gs. Topics that were suggested, <strong>in</strong>cluded,<br />

psychopharmacology, new treatments, topics <strong>on</strong> special<br />

populati<strong>on</strong>s, and an update <strong>on</strong> <strong>the</strong> new DSM-V. Well, your<br />

wish is our command and to that end <strong>the</strong> chapter will<br />

Psychiatric-Mental Health Chapter<br />

Announces Dementia Workshop<br />

On Saturday, June 22 <strong>the</strong> Psychiatric-Mental Health<br />

Chapter <strong>of</strong> SCNA will present a workshop “Dementia:<br />

What <strong>Nurses</strong> Need to Know about Types, Behavioral<br />

Strategies and Meds.” <strong>The</strong> speaker will be Barbara<br />

Barham, RN, MS, CTS. Barbara is an advanced practice<br />

psychiatric nurse with c<strong>on</strong>siderable experience work<strong>in</strong>g<br />

with pers<strong>on</strong>s affected by dementia. She presently has<br />

a small private practice devoted to crisis resp<strong>on</strong>se<br />

and dementia issues. She has been <strong>in</strong>volved with <strong>the</strong><br />

Alzheimer’s Associati<strong>on</strong> <strong>in</strong> South Carol<strong>in</strong>a for about 20<br />

years, serv<strong>in</strong>g <strong>on</strong> <strong>the</strong> Board <strong>of</strong> Directors, Public Policy<br />

Committee and as an educati<strong>on</strong>al resource. She has also<br />

been a volunteer with <strong>the</strong> American Red Cross for 6 years,<br />

serv<strong>in</strong>g as chapter leads for Health Services, Disaster<br />

Mental Health Services, and an <strong>in</strong>structor. She currently is<br />

also <strong>the</strong> State Nurse Liais<strong>on</strong> for SC and Western NC with<br />

Red Cross, a role to support nurses <strong>in</strong> Red Cross.<br />

<strong>The</strong> workshop will be held at SCNA, 1821 Gadsden<br />

St., Columbia, SC 29201. Registrati<strong>on</strong> will beg<strong>in</strong> at 9:30<br />

AM. C<strong>of</strong>fee and light refreshments will be <strong>of</strong>fered and <strong>the</strong><br />

program will be from 10 AM-12 No<strong>on</strong>. 2 CE credits will<br />

be <strong>of</strong>fered and I hour <strong>of</strong> pharm CE credit. <strong>The</strong> cost is $35<br />

for SCNA members and $50 for n<strong>on</strong>-members. Based <strong>on</strong><br />

Palmetto Gold Gala – 2013<br />

April 6, 2013<br />

It is time for <strong>the</strong> 12th annual Palmetto Gold Nurse<br />

Recogniti<strong>on</strong> and Scholarship Program Gala. <strong>The</strong> gala<br />

is set for April 6, 2013 at <strong>the</strong> Columbia Metropolitan<br />

C<strong>on</strong>venti<strong>on</strong> Center.<br />

<strong>The</strong> Palmetto Gold Nurse Recogniti<strong>on</strong> and Scholarship<br />

Program is under <strong>the</strong> auspices <strong>of</strong> <strong>the</strong> South Carol<strong>in</strong>a<br />

<strong>Nurses</strong> Foundati<strong>on</strong>, a 501-C 3 organizati<strong>on</strong> whose missi<strong>on</strong><br />

is to promote high standards <strong>of</strong> health care by <strong>in</strong>sur<strong>in</strong>g<br />

<strong>the</strong> advancement <strong>of</strong> <strong>the</strong> nurs<strong>in</strong>g pr<strong>of</strong>essi<strong>on</strong> through<br />

scholarships, grants, and programs <strong>of</strong> excellence. <strong>The</strong><br />

purpose <strong>of</strong> <strong>the</strong> Palmetto Gold program is to annually<br />

salute 100 registered nurses that exemplify excellence<br />

<strong>in</strong> nurs<strong>in</strong>g practice and commitment to <strong>the</strong> pr<strong>of</strong>essi<strong>on</strong>. A<br />

sec<strong>on</strong>dary purpose is to provide scholarships to registered<br />

nurse students ensur<strong>in</strong>g an adequate supply <strong>of</strong> nurses for<br />

<strong>the</strong> future.<br />

Palmetto Gold orig<strong>in</strong>ated <strong>in</strong> 2001 when a coaliti<strong>on</strong><br />

<strong>of</strong> nurse leaders from major nurs<strong>in</strong>g organizati<strong>on</strong>s<br />

came toge<strong>the</strong>r to plan a strategy for showcas<strong>in</strong>g <strong>the</strong><br />

many c<strong>on</strong>tributi<strong>on</strong>s that nurse’s make to <strong>the</strong> health care<br />

pr<strong>of</strong>essi<strong>on</strong>. <strong>The</strong> organizati<strong>on</strong>s <strong>in</strong>clude South Carol<strong>in</strong>a<br />

<strong>Nurses</strong> Foundati<strong>on</strong>, South Carol<strong>in</strong>a <strong>Nurses</strong> Associati<strong>on</strong>s,<br />

South Carol<strong>in</strong>a Organizati<strong>on</strong> <strong>of</strong> Nurse Leaders, South<br />

Net proceeds generated from <strong>the</strong> gala even<strong>in</strong>g are used<br />

South Carol<strong>in</strong>a registered nurse educati<strong>on</strong> programs and<br />

to build <strong>the</strong> Palmetto Gold Scholarship Fund. Palmetto<br />

available for purchase for any bus<strong>in</strong>ess or <strong>in</strong>dividual<br />

• In 2014, 32 milli<strong>on</strong> <strong>in</strong>dividuals will ga<strong>in</strong> access to<br />

health <strong>in</strong>surance under <strong>the</strong> Affordable Care Act.<br />

For <strong>the</strong> next three decades milli<strong>on</strong>s <strong>of</strong> new<br />

patients will age <strong>in</strong>to Medicare.<br />

• <strong>The</strong>se forces will cause <strong>the</strong> demand for primary<br />

care to sky-rocket. (CBO estimates with respect to<br />

<strong>the</strong> Affordable Care Act, Medicare Trustees<br />

Report)<br />

• Remov<strong>in</strong>g barriers to APRN practice will<br />

<strong>in</strong>crease access to care and, based <strong>on</strong> recent data,<br />

lower costs.<br />

• In 2011, 100,000 APRNs participated <strong>in</strong><br />

Medicare, provid<strong>in</strong>g $2.4 billi<strong>on</strong> <strong>in</strong> services to<br />

10.4 milli<strong>on</strong> fee-for-service beneficiaries (30<br />

percent <strong>of</strong> <strong>the</strong> total). If those services had been<br />

obta<strong>in</strong>ed from physicians, Medicare would have<br />

paid $340 milli<strong>on</strong> more and <strong>the</strong> beneficiaries<br />

would have been charged an additi<strong>on</strong>al $85<br />

milli<strong>on</strong>. (CMS data)<br />

• APRNs provide quality care and are cost<br />

effective.<br />

• A great example is <strong>the</strong> grow<strong>in</strong>g numbers <strong>of</strong><br />

women choos<strong>in</strong>g to have a midwife attend<br />

<strong>the</strong>ir birth – and <strong>the</strong> fact that women who deliver<br />

with midwives have fewer C-secti<strong>on</strong>s and higher<br />

breastfeed<strong>in</strong>g rates po<strong>in</strong>t to <strong>the</strong> cost effectiveness<br />

<strong>of</strong> midwifery services.<br />

• A 2011 Agency for <strong>Healthcare</strong> Care Research<br />

and Quality (AHRQ) brief found that <strong>the</strong> cost <strong>of</strong><br />

a cesarean delivery was almost $2,000 higher<br />

than a vag<strong>in</strong>al delivery. C<strong>on</strong>sider<strong>in</strong>g <strong>the</strong>re are<br />

approximately four milli<strong>on</strong> births per year, and<br />

32 percent are delivered by C-secti<strong>on</strong>, that is a<br />

total <strong>of</strong> $2.5 billi<strong>on</strong> per year.<br />

• Reduc<strong>in</strong>g this number by even just five percent<br />

by <strong>in</strong>creas<strong>in</strong>g access to midwifery care for lowrisk<br />

pregnancies would result <strong>in</strong> about $128<br />

milli<strong>on</strong> <strong>in</strong> sav<strong>in</strong>gs <strong>in</strong> hospitals al<strong>on</strong>e.<br />

<strong>The</strong> Board <strong>of</strong> Nurs<strong>in</strong>g <strong>of</strong> South Carol<strong>in</strong>a acknowledges<br />

that it is with<strong>in</strong> <strong>the</strong> role and scope <strong>of</strong> resp<strong>on</strong>sibility <strong>of</strong><br />

<strong>the</strong> RN to perform <strong>the</strong> follow<strong>in</strong>g epidural, <strong>in</strong>tra<strong>the</strong>cal or<br />

peripheral nerve <strong>the</strong>rapies:<br />

ANALGESIA/ANESTHESIA FOR ACUTE OR<br />

CHRONIC PAIN RELIEF:<br />

1. Adm<strong>in</strong>ister medicati<strong>on</strong> (opiates, local anes<strong>the</strong>tics,<br />

steroids, alpha-ag<strong>on</strong>ist, or comb<strong>in</strong>ati<strong>on</strong>s <strong>the</strong>re<strong>of</strong>) to<br />

<strong>the</strong> epidural and <strong>in</strong>tra<strong>the</strong>cal space and peripheral<br />

nerve with <strong>the</strong> use <strong>of</strong> an electr<strong>on</strong>ic pump, <strong>in</strong>fusi<strong>on</strong><br />

reservoir or by direct re-bolus exclusive <strong>of</strong> <strong>the</strong><br />

adm<strong>in</strong>istrati<strong>on</strong> <strong>of</strong> <strong>the</strong> test dose or <strong>in</strong>itial dose <strong>of</strong><br />

medicati<strong>on</strong> to determ<strong>in</strong>e correct ca<strong>the</strong>ter or <strong>in</strong>fusi<strong>on</strong><br />

device placement which is adm<strong>in</strong>istered by <strong>the</strong><br />

physician, certified registered nurse anes<strong>the</strong>tist or<br />

anes<strong>the</strong>siologist.<br />

2. <strong>The</strong> RN with tra<strong>in</strong><strong>in</strong>g and competency may assist<br />

<strong>the</strong> Anes<strong>the</strong>siologist/ Certified Registered Nurse<br />

Anes<strong>the</strong>tist (CRNA) with <strong>the</strong> adm<strong>in</strong>istrati<strong>on</strong> <strong>of</strong> a<br />

peripheral nerve block with <strong>the</strong> Anes<strong>the</strong>siologist/<br />

CRNA present and manag<strong>in</strong>g <strong>the</strong> needle<br />

placement. <strong>The</strong> RN may push <strong>the</strong> medicati<strong>on</strong> at <strong>the</strong><br />

directi<strong>on</strong> <strong>of</strong> <strong>the</strong> Anes<strong>the</strong>siologist/ CRNA while <strong>the</strong><br />

Anes<strong>the</strong>siologist/ CRNA is hold<strong>in</strong>g and manag<strong>in</strong>g <strong>the</strong><br />

needle placement.<br />

3. M<strong>on</strong>itor, ma<strong>in</strong>ta<strong>in</strong>, regulate, and/or term<strong>in</strong>ate a<br />

c<strong>on</strong>t<strong>in</strong>uous epidural, <strong>in</strong>tra<strong>the</strong>cal or peripheral nerve<br />

<strong>in</strong>fusi<strong>on</strong> <strong>of</strong> medicati<strong>on</strong>s (opiates, local anes<strong>the</strong>tics,<br />

steroids, alpha-ag<strong>on</strong>ist, or comb<strong>in</strong>ati<strong>on</strong>s <strong>the</strong>re<strong>of</strong>) as<br />

ordered by a physician, and with<strong>in</strong> <strong>the</strong> established<br />

guidel<strong>in</strong>es, policies, and procedures formulated<br />

with <strong>in</strong>put and approval <strong>of</strong> licensed physicians,<br />

anes<strong>the</strong>siologists, and/or certified registered nurse<br />

anes<strong>the</strong>tists. In home care, physician support and<br />

supervisi<strong>on</strong> may be available via telecommunicati<strong>on</strong><br />

systems.<br />

4. Attach <strong>in</strong>fusi<strong>on</strong> tub<strong>in</strong>g and devices to epidural,<br />

<strong>in</strong>tra<strong>the</strong>cal or peripheral nerve ca<strong>the</strong>ters <strong>in</strong> place<br />

(and placement verified), as ordered by physician<br />

and under <strong>the</strong> supervisi<strong>on</strong> <strong>of</strong> a licensed physician or<br />

certified registered nurse anes<strong>the</strong>tist.<br />

This applies, but is not limited to, situati<strong>on</strong>s <strong>in</strong> which:<br />

a. Patient requires acute or chr<strong>on</strong>ic pa<strong>in</strong><br />

management.<br />

b. Patient requires post-surgical pa<strong>in</strong> management.<br />

c. Physician provider verified correct ca<strong>the</strong>ter<br />

placement<br />

d. Patient’s vital signs are stabilized.<br />

e. Patient’s anes<strong>the</strong>sia/analgesic level is established.<br />

MANAGEMENT AND MONITORING OF<br />

INTRAPARTUM EPIDURAL ANESTHESIA/<br />

ANALGESIA:<br />

1. M<strong>on</strong>itor <strong>the</strong> <strong>in</strong>trapartum patient receiv<strong>in</strong>g epidural<br />

anes<strong>the</strong>sia/analgesia provided a licensed physician,<br />

anes<strong>the</strong>siologist and/or certified registered nurse<br />

anes<strong>the</strong>tist is present and resp<strong>on</strong>sible (IMMEDIATELY<br />

AVAILABLE ON SITE.)<br />

2. Term<strong>in</strong>ate an epidural <strong>in</strong>fusi<strong>on</strong> with immediate<br />

notificati<strong>on</strong> <strong>of</strong> <strong>the</strong> attend<strong>in</strong>g physician, certified<br />

registered nurse anes<strong>the</strong>tist or certified nurse midwife.<br />

This m<strong>on</strong>itored care is <strong>on</strong>ly to be d<strong>on</strong>e follow<strong>in</strong>g<br />

stabilizati<strong>on</strong> <strong>of</strong> vital signs after ei<strong>the</strong>r bolus <strong>in</strong>jecti<strong>on</strong><br />

or establishment <strong>of</strong> c<strong>on</strong>t<strong>in</strong>uous pump <strong>in</strong>fusi<strong>on</strong> by<br />

physician, anes<strong>the</strong>siologist or certified registered<br />

nurse anes<strong>the</strong>tist.<br />

3. <strong>The</strong> RN may replace empty <strong>in</strong>fusi<strong>on</strong> syr<strong>in</strong>ges or bags<br />

with prepared soluti<strong>on</strong>s provided that <strong>the</strong> soluti<strong>on</strong> is<br />

verified by a sec<strong>on</strong>d licensed nurse. <strong>The</strong> RN MAY NOT<br />

prepare soluti<strong>on</strong>s for <strong>in</strong>fusi<strong>on</strong>, alter <strong>the</strong> rate (<strong>in</strong>crease<br />

or decrease), <strong>in</strong>ject, bolus, re-bolus <strong>the</strong> anes<strong>the</strong>tic/<br />

analgesic <strong>in</strong>fusi<strong>on</strong>. <strong>The</strong> registered nurse may not <strong>in</strong>sert<br />

<strong>the</strong> ca<strong>the</strong>ter, positi<strong>on</strong> or repositi<strong>on</strong>, or flush to ma<strong>in</strong>ta<strong>in</strong><br />

patency.<br />

REMOVAL OF CATHETER:<br />

<strong>The</strong> RN may remove epidural or peripheral nerve<br />

(not <strong>in</strong>tra<strong>the</strong>cal) ca<strong>the</strong>ters, provided <strong>in</strong>serti<strong>on</strong> was<br />

documented to be uncomplicated.<br />

<strong>The</strong> Board recognizes that <strong>the</strong>se resp<strong>on</strong>sibilities<br />

are an additi<strong>on</strong>al act for <strong>the</strong> RN. <strong>The</strong> agency policies<br />

and procedures should address how <strong>the</strong> agency will<br />

assure a physician or Advanced Practice Registered<br />

Nurse authorizes to adm<strong>in</strong>ister epidural and <strong>in</strong>tra<strong>the</strong>cal<br />

<strong>the</strong>rapy/procedures. Medicati<strong>on</strong>s to be adm<strong>in</strong>istered by<br />

<strong>the</strong> RN and dose ranges are to be listed <strong>in</strong> <strong>the</strong> policies<br />

and procedures. <strong>The</strong> nurs<strong>in</strong>g department is advised<br />

to obta<strong>in</strong> patient specific written medical orders which<br />

<strong>in</strong>dicate mechanical and physiological parameters to be<br />

m<strong>on</strong>itored and reported to <strong>the</strong> physician.<br />

This statement is an advisory op<strong>in</strong>i<strong>on</strong> <strong>of</strong> <strong>the</strong> Board<br />

<strong>of</strong> Nurs<strong>in</strong>g as to what c<strong>on</strong>stitutes competent and safe<br />

nurs<strong>in</strong>g practice.<br />

REPORTING MISCONDUCT AND VIOLATIONS OF<br />

THE NURSE PRACTICE ACT<br />

It has come to <strong>the</strong> attenti<strong>on</strong> <strong>of</strong> <strong>the</strong> S.C. Board <strong>of</strong><br />

Nurs<strong>in</strong>g that <strong>the</strong>re is reluctance <strong>on</strong> <strong>the</strong> part <strong>of</strong> some<br />

employers to report violati<strong>on</strong>s <strong>of</strong> <strong>the</strong> Nurse Practice Act<br />

by <strong>the</strong>ir employees.<br />

S. C. Code Ann. § 40-33-111(A) <strong>of</strong> <strong>the</strong> Nurse Practice<br />

Act states that:<br />

(A) An employer, <strong>in</strong>clud<strong>in</strong>g an agency, or supervisor<br />

<strong>of</strong> nurses, shall report any <strong>in</strong>stances <strong>of</strong> <strong>the</strong> misc<strong>on</strong>duct<br />

or <strong>the</strong> <strong>in</strong>capacities described <strong>in</strong> Secti<strong>on</strong> 40-33-110<br />

to <strong>the</strong> State Board <strong>of</strong> Nurs<strong>in</strong>g not more than fifteen<br />

bus<strong>in</strong>ess days, except<strong>in</strong>g Saturdays, Sundays, and<br />

legal holidays, from <strong>the</strong> discovery <strong>of</strong> <strong>the</strong> misc<strong>on</strong>duct<br />

or <strong>in</strong>capacity. A nurse supervisor who fails to timely<br />

report <strong>the</strong> misc<strong>on</strong>duct or <strong>in</strong>capacity may be subject to<br />

discipl<strong>in</strong>ary acti<strong>on</strong> and civil sancti<strong>on</strong>s as provided for <strong>in</strong><br />

Secti<strong>on</strong> 40-33-120. An employer who is not licensed by<br />

Name: Lisa Cox Irv<strong>in</strong> Schafer, RN, MSN, NEA-BC<br />

Pr<strong>of</strong>essi<strong>on</strong>al Background/Experience: I have<br />

been <strong>the</strong> Vice President and Senior Nurse Executive<br />

for Roper Hospital <strong>in</strong> Charlest<strong>on</strong>, South Carol<strong>in</strong>a s<strong>in</strong>ce<br />

2005. Prior to that promoti<strong>on</strong>, I was <strong>the</strong> Director <strong>of</strong><br />

Cardiovascular Cl<strong>in</strong>ical Services at Roper for two years.<br />

<strong>The</strong> rest <strong>of</strong> my career <strong>in</strong>cludes six years as an ADN<br />

nurs<strong>in</strong>g faculty member and nearly two decades <strong>of</strong><br />

staff and management positi<strong>on</strong>s at MUSC, primarily <strong>in</strong><br />

cardiovascular, critical care and trauma services.<br />

Why did you want to be <strong>on</strong> <strong>the</strong> Board? I was<br />

h<strong>on</strong>ored to have <strong>the</strong> opportunity to be a member <strong>of</strong> <strong>the</strong><br />

State Board <strong>of</strong> Nurs<strong>in</strong>g. I believe <strong>the</strong> role <strong>of</strong> <strong>the</strong> State<br />

Board will be particularly critical <strong>in</strong> <strong>the</strong> upcom<strong>in</strong>g years as<br />

nurses face significant changes <strong>in</strong> health care delivery <strong>of</strong><br />

our nati<strong>on</strong>. With nati<strong>on</strong>al mandates to decrease costs and<br />

improve health outcomes, nurs<strong>in</strong>g is positi<strong>on</strong>ed to have a<br />

significant impact <strong>on</strong> <strong>the</strong> future health <strong>of</strong> citizens <strong>of</strong> our<br />

state and country. I thought it was important to have a<br />

nurse leader <strong>on</strong> <strong>the</strong> board who is <strong>in</strong>timately <strong>in</strong>volved with<br />

<strong>the</strong> challenges <strong>of</strong> <strong>the</strong> daily practice <strong>of</strong> nurs<strong>in</strong>g.<br />

What do you like most about work<strong>in</strong>g with SC<br />

BON? I have learned a great deal more about nurs<strong>in</strong>g<br />

educati<strong>on</strong> programs, <strong>the</strong> legislative process, due<br />

process, and <strong>the</strong> laws govern<strong>in</strong>g our practice. I have a<br />

much greater appreciati<strong>on</strong> for <strong>the</strong> processes that protect<br />

both <strong>the</strong> public and <strong>the</strong> nurs<strong>in</strong>g workforce and LLR’s<br />

commitment to c<strong>on</strong>t<strong>in</strong>uous improvement.<br />

Hobbies: I spend my spare time travel<strong>in</strong>g, runn<strong>in</strong>g,<br />

play<strong>in</strong>g tennis and hik<strong>in</strong>g <strong>in</strong> <strong>the</strong> mounta<strong>in</strong>s. Last February<br />

my husband and I hiked to <strong>the</strong> summit <strong>of</strong> Mt. Kilimanjaro,<br />

<strong>the</strong> tallest peak <strong>in</strong> Africa.<br />

requires that all licensees notify <strong>the</strong> Board <strong>in</strong> writ<strong>in</strong>g with<strong>in</strong> 15 days <strong>of</strong> any address<br />

change. So you do not miss important time-sensitive <strong>in</strong>formati<strong>on</strong> from <strong>the</strong> Board, such<br />

as your courtesy renewal notice, audit notice or o<strong>the</strong>r important licensure <strong>in</strong>formati<strong>on</strong>,<br />

be sure to notify <strong>the</strong> Board immediately whenever you change your address. Failure to<br />

notify <strong>the</strong> Board <strong>of</strong> an address change may result <strong>in</strong> a public reprimand and $500 civil<br />

RETURNED CHECKS<br />

funds <strong>in</strong> your account to cover your payment (paper or electr<strong>on</strong>ic check or credit card)<br />

and that <strong>the</strong> payment has cleared before clos<strong>in</strong>g <strong>the</strong> account. Secti<strong>on</strong> 40-1-50(G) <strong>of</strong> <strong>the</strong><br />

South Carol<strong>in</strong>a Code <strong>of</strong> Laws states that a license shall be suspended if a fee payment<br />
