Contact Lens Formulary Expansion

OptumHealth Vision (Spectera) has announced that its Contact Lens Formulary is
expanding as of June 1, 2012. Due to this formulary update, you can now offer a
wider variety of affordable contact lenses to your OptumHealth Vision patients. If
you have not yet received the details of the changes from OptumHealth Vision,
please contact your OptumHealth Vision customer service representative.
NOTE
This document is applicable to AcuityLogic On Demand and was
last updated on June 5, 2012. This document is subject to change at
any time without notice.
Formulary
Updates
OptumHealth Vision (Spectera) is updating its Contact Lens Formulary to include
more contact lenses. The following changes to the formulary go into effect on
June 1, 2012:
• Acuvue Oasys with Hydraclear contact lenses will be added to the Covered
Disposable Selection category in the formulary.
• The following brands of daily wear contact lenses will be added to the
Covered Disposable Selection category in the formulary:
Dailies AquaComfort Plus 30 pack
Focus Dailies Toric 30 pack
Focus Dailies Progressives 30 pack
Proclear 1-Day 30 pack
Acuvue 1-Day Moist 30 pack
• Private Label Equivalent contact lenses will be added to the formulary. All
private label versions of formulary lenses must be submitted as Covered
Selection lenses. (Refer to your OptumHealth Vision notice for a list of all
eligible private label contact lenses.)
• Bausch & Lomb Pure Vision monthly replacement contact lenses will no
longer be included in the formulary.
In order to accommodate the new changes, Eyefinity will be updating all of your
OptumHealth Vision (Spectera) plans over the next few weeks. You do not need to
make any changes to your system. You will be notified by Eyefinity after all
updates to your OptumHealth Vision plans are complete.

Formulary Updates
Until the updates to your OptumHealth Vision plans are complete, please contact
Eyefinity Customer Care if you have a patient who wishes to purchase one of the
formulary lenses listed above. A Customer Care representative will then add the
lens for you to your version of the specific OptumHealth Vision Plan.
If you have any questions or have a patient who wishes to purchase one of the
formulary lenses listed above, contact Eyefinity Customer Care at 800.942.5353,
option 2, or AcuityLogicsupport@eyefinity.com.
NOTE
Effective July 1, 2012, OptumHealth Vision will require that all contact
lenses included on claims must be submitted with new contact lens
modifier codes. Eyefinity will communicate any necessary
modifications to the AcuityLogic system to you at a later date.
