Required Forms o The Ohio Administrative Code requires the use of the following PERRP recordkeeping forms: • PERRP Form 300P Log; • PERRP Form 300AP Summary; • PERRP Form 301P Incident Report. o BWC First Report of Injury (FROI) is “equivalent” to 301P. o PERRP has developed its own set of recordkeeping forms that are “equivalent” to the OSHA forms. 10 PERRP Recordkeeping
