group. He was observed by several jumpcrs ln the alr but he did<br />

not enter the formatlon, There were no reports of any unusuaf<br />

occurancss durl,ng the jump. After the formatlon broke, the deceased<br />

was obseryed by rellable, experiencad jurupera on the ground to be<br />

tumbling until an altitude of approxioately J.J0-200', at v{hlch<br />

tlne ha got stable and deployed the reeerve canopy. I@pact occurred<br />

observed back loops. She appeared to be ln control at thls<br />

also have grabbed both. The front loop obeerved at 1000-1500,<br />

ft could be when the subject gaw how lov she wag and grabbed<br />

the orange colored sln9le polnt release handle rrlthout looklng or<br />

thinklng it was the reserve handle, She looked as 1f ghe made a<br />

two handed pul1 due to the velcro resistance whlch pulled her<br />

lnto the front loop. l{hen she Etabled out I think ahe t}rought the<br />

reserve failed to deploy and she then went back to lhe Hand<br />

deploy whlch she deployed successfully or she had enough of lt<br />

out that it was drug out by the wind blast. If sho dld actlvato<br />

the hand deploy at that altltude I tilnk ahe eould have released<br />

the 3 ring handle and the location of the handle down ulnd of tl.e<br />

impact polnt would supporC this possiblltty.<br />

I. Encourage manufactures to neke a deslgn change to all hand<br />

deploy pockets to allow extrae'trr' of tie pllot chute ln a<br />

fonrard puII, antl ln the event of a pocket Iock sltuatlon, allov<br />

tie pouch to busi, open, perhaps through the use of velcro. Irve<br />

seen reports of other fatalllies attributed to thlB problem.<br />

Irve also witheiesed totabcaused by a pouch lock thal would have<br />

never happened if the pouch could break open. If the manufacturea<br />

deem it too expenslve to lncorporated such a change tnto thetr<br />

systems, then at least get.them to present It to the<br />

publlc Jurnptng<br />

so it can be done on an J4dj,vidual baels legal]y.<br />

2. A working AOD wou.ld have prevented thls faCattty. The prlce<br />

availab]eAOD's effectively keeps them out of the nande ana off<br />

the rigs of people like t}te deceased; the developtng sport parachutist,<br />

at a time when they need It t}le mosh (i,e. beginnlng RW, jumping<br />

lew gear, long perlods of inactivity etc). Hov can a manufacturer<br />

jusCify selling an AOD unlt for twj.ce lrhat it corito to produce?<br />

[The benefits of a capltallst society)? perhaps-the USPA safety<br />

and tralning coNnlttee should take a hard look at the countrleg<br />

that require AODrs on aII Jumps, Couldn't USPA use its lobbylng<br />

Powers to produce an affordable unit (I00 dollare) that USPA<br />

would endorse, I have 96? jumps made over a.perlod of 17 years.<br />

I i'ntend to lncorporate an AOD on my systeF as soon aa I can<br />

Events: Deceased exited the aircraft at 9500r on the first<br />

pass over the Dz with a second jumper following a few seconds<br />

behind. As observed from the A/C and as confirmed by ground<br />

witnesses, only one canopy opened, Deceased vas not observed<br />

either from the gr.gund or the A,/C during the latter portlon<br />

A search was drganized eith the body being discovered tuo hours<br />

being folIoued, The second case involved uslng a<br />

stopwacch as the insirumeng for a 15 sec. delay while ln a<br />

tj.ght frog position and vearing an AoD, He exPerienced<br />

simrrltaneous deployment of the main and reserve. He cutaway<br />

the main, Ianding under the reserve wlth no further<br />

inc ident.<br />

conclusions: Positioning of all handles uould indicate a<br />

"no attempt to pu]I" sltuaLion, Dark 9o99Ies in combination<br />

with lorr Iight level vould have contrlbuted to a Poor height<br />

perception with no horizon visibl.e. Log book indicates that<br />

previous 20 jumps ranged from 0 sec. to {5 sec. delay in a<br />

random pdttern, inhibiting the develoPment of a reliable<br />

time sense, Coroner's autopsy may provide further information.<br />
