The coord<strong>in</strong>ation of falls prevention services <strong>in</strong> <strong>Southern</strong> <strong>Adelaide</strong>,<br />

<strong>in</strong>clud<strong>in</strong>g referrals to the RGH <strong>Falls</strong> Assessment Cl<strong>in</strong>ic, is now be<strong>in</strong>g<br />

undertaken by the <strong>Southern</strong> Community <strong>Falls</strong> <strong>Prevention</strong> Team<br />

Who is the service for?<br />

This service is for adults liv<strong>in</strong>g <strong>in</strong> the<br />

community who have had a fall or who<br />

are at risk of fall<strong>in</strong>g and who would<br />

benefit from coord<strong>in</strong>ation of falls<br />

prevention services.<br />

What does the service offer?<br />

Common Entry po<strong>in</strong>t for triag<strong>in</strong>g and<br />

screen<strong>in</strong>g of falls prevention referrals:<br />

> To identify the level of falls risk, and<br />

urgency of response required<br />

> To ensure correct service match<strong>in</strong>g and<br />

referral to appropriate services.<br />

What are some of the<br />

Service Options available?<br />

> RGH <strong>Falls</strong> Assessment Cl<strong>in</strong>ic<br />

(if <strong>in</strong>dicated)<br />

> In home falls risk screen & assessment<br />

as part of the Pathways to<br />

Independence Program<br />

> L<strong>in</strong>kage with community services and<br />

programs<br />

> L<strong>in</strong>kage with exist<strong>in</strong>g services eg DVA,<br />

community packages<br />

> Follow-up and monitor<strong>in</strong>g<br />

> GP Practices<br />

> South Australian Ambulance Service<br />

> Hospitals <strong>in</strong> the <strong>Southern</strong> <strong>Adelaide</strong><br />

<strong>Health</strong> Area<br />

> Selected Community Agencies.<br />
