The RGH <strong>Falls</strong> Assessment Cl<strong>in</strong>ic provides specialised comprehensive<br />
multidiscipl<strong>in</strong>ary assessments for older clients with more complex<br />
needs around falls prevention.<br />
Who is this service for?<br />
The program is for older adults liv<strong>in</strong>g <strong>in</strong><br />
the community who have more complex<br />
issues around falls and frailty who<br />
would benefit from a comprehensive<br />
assessment and coord<strong>in</strong>ation of falls<br />
prevention services.<br />
What does the service offer?<br />
Specialised comprehensive Geriatrician<br />
led multidiscipl<strong>in</strong>ary assessment:<br />
> To identify falls risk factors<br />
> Offer services/<strong>in</strong>terventions to address<br />
these risk factors<br />
> Access to a specialised Day<br />
Rehabilitation Service (FIT Program)<br />
> To l<strong>in</strong>k <strong>in</strong> with other community<br />
services as required<br />
> Follow up with geriatrician as required<br />

> GP Practices<br />
> South Australian Ambulance Service<br />
> Hospitals <strong>in</strong> the <strong>Southern</strong><br />
Metropolitan Area<br />
> Selected Community Agencies.<br />
What is the criteria for<br />
referral?<br />
> Need for more detailed specialist<br />
medical assessment<br />
> Uncerta<strong>in</strong> diagnosis<br />
> Unexpla<strong>in</strong>ed falls/gait disturbances<br />
> Untreated osteoporosis<br />
> Decl<strong>in</strong><strong>in</strong>g function with mod-severe<br />
disability<br />
> Unexpla<strong>in</strong>ed syncope or collapse<br />
> Complex medication issues<br />
> Cognitive issues affect<strong>in</strong>g mobility<br />
