3.1 Challenges<br />

For trust pr<strong>of</strong>essional staff to demonstrate a clear trail from <strong>the</strong> assessment<br />

outcomes to <strong>the</strong> interventions / services provided for carers.<br />

3.2 Recommendations<br />

8 Trusts should encourage wider participation <strong>of</strong> health and social<br />

care pr<strong>of</strong>essionals in <strong>the</strong> process <strong>of</strong> needs assessment.<br />

9 Trusts should encourage staff to make all attempts to ensure that<br />

where possible <strong>the</strong> carer retains a copy <strong>of</strong> <strong>the</strong> assessment.<br />

10 Trusts should review if <strong>the</strong>y are acting in accordance with<br />

Principle 7 <strong>of</strong> <strong>the</strong> Data Protection Act, in cases where <strong>the</strong>y retain<br />

a combined record <strong>of</strong> <strong>the</strong> carer and cared for person in one file<br />

and document clearly why <strong>the</strong>y need to do so.<br />
