A person-centred process whereby <strong>the</strong> needs <strong>of</strong> an individual<br />

are identified and <strong>the</strong>ir impact on daily living and quality <strong>of</strong> life<br />

is evaluated, undertaken with <strong>the</strong> individual, his/her carer and<br />

relevant pr<strong>of</strong>essionals.<br />

A health care pr<strong>of</strong>essional, typically a nurse or social worker,<br />

who arranges, monitors, or coordinates long-term care services<br />

(also referred to as a care coordinator or case manager). A<br />

care manager may also assess a patient's needs and develop<br />

a plan <strong>of</strong> care, subject to approval by <strong>the</strong> patient's physician<br />

Is a whole concept which embraces <strong>the</strong> key functions <strong>of</strong>: case<br />

finding; case screening; undertaking proportionate; personcentred<br />

involves ascertaining <strong>the</strong> level and type <strong>of</strong> support required to<br />

meet those needs, and <strong>the</strong> objectives and potential outcomes<br />

that can be achieved.<br />

People who, without payment, provide help and support to a<br />

family member or friend who may not be able to manage at<br />

home without this help because <strong>of</strong> frailty, illness or disability<br />

A care worker is someone whose job involves helping people<br />

who have particular problems or special needs, for example in<br />
