Additional considerations<br />

<strong>Language</strong> tra<strong>in</strong><strong>in</strong>g <strong>for</strong> physicians<br />

If you speak a non-English language and would like to improve your language skills, there are a variety of opportunities<br />

<strong>for</strong> further study:<br />

• <strong>Language</strong> learn<strong>in</strong>g, self-study<br />

<strong>Language</strong> tra<strong>in</strong><strong>in</strong>g CDs are useful <strong>for</strong> daily practice <strong>in</strong> the car, at home, or dur<strong>in</strong>g lunch time. Onl<strong>in</strong>e courses can be<br />

remote or real-time and can be especially useful <strong>for</strong> physicians <strong>in</strong> rural areas. A quick search of the web or a trip to<br />

your local bookstore will present you with many choices.<br />

• <strong>Language</strong> classes, <strong>in</strong> person and onl<strong>in</strong>e<br />

If you prefer a more structured language learn<strong>in</strong>g environment, try your local community college, or search the web<br />

<strong>for</strong> onl<strong>in</strong>e language classes.<br />

• Immersion<br />

The value of an immersion experience is that you are <strong>for</strong>ced to use the language <strong>in</strong>stead of just study it. <strong>Language</strong><br />

is, after all, a tool; you will learn a language best when you need the tool to accomplish a goal <strong>in</strong> which you are<br />

<strong>in</strong>terested.<br />

Most people th<strong>in</strong>k of immersion as requir<strong>in</strong>g one to leave the country. While it’s true that a prolonged stay <strong>in</strong> Costa<br />

Rica will certa<strong>in</strong>ly hone your Spanish, ongo<strong>in</strong>g volunteer work at a social service program <strong>in</strong> a Spanish-dom<strong>in</strong>ant<br />

neighborhood of your town or city will also give you plenty of l<strong>in</strong>guistic practice, and with the community which<br />

your medical practice serves as well. There are also immersion courses <strong>in</strong> the US, <strong>for</strong> example a Spanish language<br />

immersion program has been developed <strong>in</strong> Chicago.<br />

Interpreter tra<strong>in</strong><strong>in</strong>g<br />

Anyone who <strong>in</strong>terprets, whether a bil<strong>in</strong>gual staff member, a contractor, or an agency <strong>in</strong>terpreter, should receive<br />

basic <strong>in</strong>terpreter tra<strong>in</strong><strong>in</strong>g be<strong>for</strong>e provid<strong>in</strong>g this service. While be<strong>in</strong>g bil<strong>in</strong>gual is a prerequisite of be<strong>in</strong>g a good <strong>in</strong>terpreter,<br />

it is not enough. Interpret<strong>in</strong>g <strong>in</strong>volves a complex series of skills <strong>in</strong>volv<strong>in</strong>g understand<strong>in</strong>g a message and<br />

recreat<strong>in</strong>g the same message <strong>in</strong> a different l<strong>in</strong>guistic and cultural context. It also <strong>in</strong>cludes skills <strong>for</strong> communicat<strong>in</strong>g<br />

directly with the people <strong>for</strong> whom he/she is <strong>in</strong>terpret<strong>in</strong>g, and those related to stay<strong>in</strong>g <strong>in</strong> the background, support<strong>in</strong>g<br />

the patient/physician relationship, manag<strong>in</strong>g the flow of the session, and identify<strong>in</strong>g possible culturally-based misunderstand<strong>in</strong>gs.<br />

Tra<strong>in</strong>ed <strong>in</strong>terpreters make fewer errors, know how to stay <strong>in</strong> the background, and help the<br />

encounter go faster and more smoothly.<br />

Tra<strong>in</strong><strong>in</strong>g programs <strong>for</strong> <strong>for</strong>eign language <strong>in</strong>terpreters vary <strong>in</strong> length, from short 24-hour courses to year-long college<br />

programs. At the time of this writ<strong>in</strong>g, there are not yet national standards regard<strong>in</strong>g what constitutes an acceptable<br />

tra<strong>in</strong><strong>in</strong>g program <strong>for</strong> health care <strong>in</strong>terpreters, but there is general agreement that a 40-hour course is a basic <strong>in</strong>troduction.<br />

Courses shorter than that should be considered orientation, not tra<strong>in</strong><strong>in</strong>g.<br />

1) Health Care Interpreter Tra<strong>in</strong><strong>in</strong>g <strong>in</strong> the State of Cali<strong>for</strong>nia (Roat, C. The Cali<strong>for</strong>nia Endowment, 2002), is<br />

available at http://www.calendow.org/reference/publications/cultural_competence.stm. This<br />

document <strong>in</strong>cludes a survey and list<strong>in</strong>g of tra<strong>in</strong><strong>in</strong>g programs throughout the state of Cali<strong>for</strong>nia.<br />

2) A 2004 update to the list of tra<strong>in</strong><strong>in</strong>g programs <strong>in</strong> the previous study is available from the Cali<strong>for</strong>nia<br />

Healthcare Interpreter Association at www.chia.ws. The CHIA website also has <strong>in</strong><strong>for</strong>mation about<br />

other tra<strong>in</strong><strong>in</strong>g programs accessible by click<strong>in</strong>g on “Tra<strong>in</strong><strong>in</strong>g and Test<strong>in</strong>g Resources.”
