PART II - GENERAL PROVISIONS<br />
II-1. Scope of <strong>Agreement</strong>. The <strong>Military</strong> Surface Deployment and Distribution Command<br />
(SDDC) is the executive agent for surface transportation within the Continental United States.<br />
The <strong>Military</strong> <strong>Bus</strong> <strong>Agreement</strong> maintained by SDDC establishes the terms, conditions and safety<br />
standards which the carrier will meet when providing bus transportation for Department of<br />
Defense passengers. It does not bind the Government to award movements to the carrier nor<br />
does it bind the carrier to offer service.<br />
This agreement applies to the movement of DOD passengers, baggage and impedimenta between<br />
points within the Continental United States (CONUS), or as otherwise agreed by both parties. It<br />
will become effective when signed by the parties, and will remain in effect until terminated in<br />
writing by the carrier or canceled in whole or in part by SDDC.<br />
II-2. Carrier Application. Carriers wishing to transport DOD passengers must apply for<br />
SDDC <strong>Military</strong> <strong>Bus</strong> <strong>Agreement</strong> program approval. Applicants must have a minimum of one<br />
year of operating and passenger transportation experience. Carriers must submit the following<br />
information to the SDDC approving office:<br />
a. Signed <strong>Military</strong> <strong>Bus</strong> <strong>Agreement</strong> Signature Page (page iv)<br />
b. Carriers’ Interstate Commerce Commission (ICC) or Federal Highway Administration<br />
(FHA) Certificate of Public Convenience and Necessity, and/or a copy of State Operating<br />
Certificates. Deregulated state applicants must submit a notarized statement that carrier<br />
meets all state licensing requirements.<br />
c. Interstate carriers are no longer required to have and submit a Department of<br />
Transportation Safety Fitness Rating (see Item III-3).<br />
d. Certificate of Insurance (Attachment 3). This form is required for companies not holding<br />
ICC/FHA authority and must be notarized. All other companies may submit the ACORD<br />
certificate of insurance form for SDDC’s information.<br />
e. List of Carrier Personnel to be contacted for transportation arrangements or questions<br />
ON CARRIER LETTERHEAD (Attachment 4).<br />
f. A completed Inventory Listing of Equipment ON CARRIER LETTERHEAD<br />
(Attachment 5). Please annotate any handicap accessible equipment in your<br />
inventory.<br />
g. A signed copy of the Certification Regarding a Drug/Alcohol-Free Workplace<br />
