KENMORE ® DISHWASHER RECALL CASH REBATE PROGRAM
Because your dishwasher has been identified as being included in this
recall, you must discontinue use and arrange for a free repair or
replace it as soon as possible.
Customers who wish to replace their dishwasher with a new model can use this cash
rebate certificate of $150 for Kenmore dishwashers. The recalled Kenmore dishwashers
were manufactured from September 1997 to December 2001. The model and serial
numbers are listed below:

TO RECEIVE THIS REATE, YOU MUST:
1) Complete the bottom portion of this certificate
providing your name, address and phone
number along with the rebate option you have
selected.
2) Attach the sales receipt from your new
dishwasher to this certificate. Receipts must
include your retailer’s name, your name,
address, and new Kenmore dishwasher model
number. New dishwashers
must be purchased from retail stores. This
rebate does not apply to purchases of used or
as-is dishwashers.
3) Remove the model/serial number label and
return it with the rebate package.
4) Mail the cash rebate certificate, your old
model/serial number label, and your new
dishwasher sales receipt to:

5) Your rebate check will be mailed to you in 4 to 6
weeks. The check is valid up to 90 calendar
days of issuance.
6) This rebate can be used in conjunction with any
other retail sales promotion.
