The <strong>Member</strong> ID is the same as your child's <strong>CHIP</strong> ID number <strong>and</strong> usually begins with an "A", "D",<br />

“G”, “J” or “M”, or with the numbers “0”, “1”, “2”, “3” or “4”.<br />

Each member will have a different card. Your child will get a new ID card if it gets lost, if you<br />

call us to change your child’s PCP, or if your child’s co-pays change.<br />

<strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn ID Card<br />

You will get a <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn Identification (ID) card from us when your<br />

child is enrolled in the <strong>CHIP</strong> Perinatal Program. An example of the <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate<br />

Newborn ID card is shown below.<br />

The front of the <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> <strong>and</strong> <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn ID cards show<br />

important in<strong>for</strong>mation about your child, as well as the name <strong>and</strong> phone number of your child’s<br />

PCP. <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> ID card will have co-payment in<strong>for</strong>mation, if applicable. Co-payments<br />

are not required <strong>for</strong> the <strong>CHIP</strong> Perinatal Program. The <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn<br />

ID card will show no co-payment in<strong>for</strong>mation. The back of the card has additional in<strong>for</strong>mation,<br />

as well as important phone numbers <strong>for</strong> you to call if you need help.<br />
