You will get a letter in the mail if you need to pick a new PCP <strong>for</strong> your child. You will need to<br />

pick a new PCP right away. If you do not pick a new PCP, we will pick one <strong>for</strong> your child near<br />

your home <strong>and</strong> send you a new ID card with the new PCP’s name, address <strong>and</strong> telephone number<br />

on it.<br />

To give your child the best care possible, the PCP needs to know your child’s medical history.<br />

Your child’s medical records are private <strong>and</strong> confidential. Only you, your child’s PCP, <strong>and</strong> other<br />

approved providers have a right to see them. If you change doctors, be sure to give your child’s<br />

new PCP any in<strong>for</strong>mation needed about his or her medical history.<br />

What if I Choose to Go to Another Doctor Who<br />

Is Not My/ My Child’s PCP?<br />

Most of the time, you need to go to your child’s PCP first. If you choose to go to another doctor,<br />

you may have to pay. However, there are some benefits that do not require that you go to your<br />

child’s PCP first. They include:<br />

• Emergency Care (<strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> <strong>and</strong> <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborns)<br />

• OB/GYN Care (<strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> members only)<br />

Contact <strong>Member</strong> Services toll-free at 1-888-814-2352 <strong>for</strong> more in<strong>for</strong>mation.<br />

What if my/ my child’s provider leaves <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong><br />

or <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn?<br />

If your child’s PCP leaves the <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> or <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn<br />

programs, we will send you a letter. The letter will tell you the new PCP we have chosen <strong>for</strong> you.<br />

You can choose a different PCP if you want. Call <strong>Member</strong> Services toll-free at 1-888-814-2352<br />

<strong>and</strong> tell them which PCP you want. If your child is receiving medically necessary treatments,<br />

your child will be able to stay with that PCP provider until we find another PCP in our network<br />

who can give your child the same type of care.<br />

What if I want to change health plans? Who do I call?<br />

<strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> <strong>Member</strong>s<br />
