How will I be notified if services are denied?<br />

If your services are denied, you <strong>and</strong> your doctor will receive a letter that includes the reason <strong>for</strong><br />

denial <strong>and</strong> your rights to appeal. The letter will include directions on how to file an appeal <strong>and</strong><br />

will include in<strong>for</strong>mation about how to request a review by an independent review organization.<br />

When do I have the right to request an appeal?<br />

If you don’t agree with the decision made by the health plan about a benefit or service, you may<br />

ask <strong>Parkl<strong>and</strong></strong> Community Health Plan <strong>for</strong> an appeal. You do not have a right to an appeal if the<br />

services you requested are not covered under the <strong>CHIP</strong> or <strong>CHIP</strong> Perinatal program or if a change<br />

is made to the state or federal law, which affects some or all of <strong>CHIP</strong> or <strong>CHIP</strong> Perinatal<br />

recipients.<br />

Does my request have to be in writing?<br />

Your request does not have to be in writing. You may request an appeal by calling our <strong>Member</strong><br />

Services Department at 1-888-814-2352.<br />

What are the timeframes <strong>for</strong> the appeal process?<br />

You may appeal a decision to deny services at any time after you are notified of the decision. The<br />

timeframe <strong>for</strong> the resolution of the appeal will depend on what services have been denied. If you<br />

have an emergency, are in the hospital or are already receiving services that are being limited or<br />

denied, you may call <strong>and</strong> request an expedited appeal. The expedited appeal process is described<br />

below.<br />

For a st<strong>and</strong>ard appeal, the <strong>Member</strong> Advocate will send you a letter within five (5) days of<br />

receiving the request <strong>for</strong> an Appeal to let you know that we received your request. <strong>Parkl<strong>and</strong></strong><br />

Community Health Plan will send all available in<strong>for</strong>mation to a physician who was not involved<br />

in making the initial decision. When the plan makes a decision on an appeal you made, you will<br />

receive a written response within thirty (30) days after we receive the appeal.<br />

Can someone from <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> or <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn help me file an<br />

appeal?<br />

The <strong>Member</strong> Advocate will listen to your appeal, explain the rules to you, answer your questions<br />

<strong>and</strong> see to it that you are treated fairly.<br />

What Is an Expedited Appeal?<br />

An expedited appeal is an appeal to <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> or <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn in<br />

which the decision is required quickly based on the <strong>Member</strong>’s health status, <strong>and</strong> the amount of<br />

time necessary to participate in a st<strong>and</strong>ard appeal could jeopardize the <strong>Member</strong>’s life or health or<br />

ability to attain, maintain, or regain maximum function.<br />
