How Many Times Can I Change My/ My Child’s PCP?<br />

You can change your child’s PCP up to four times a year by calling <strong>Member</strong> Services toll-free at<br />

1-888-814-2352.<br />

When Will a PCP Change Become Effective?<br />

The change will become effective on the day you make the change. A complete list of doctors <strong>and</strong><br />

clinics is found in the <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> <strong>and</strong> <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate Newborn Provider<br />

Directory. You can see or print a copy of the Provider Directory at www.<strong>Parkl<strong>and</strong></strong>HMO.com.<br />

You will get a new ID card that shows the date your child’s new PCP will be effective. The new<br />

card will also show the new PCP’s name, address <strong>and</strong> phone number on it. Changing your child’s<br />

PCP will not stop your child from getting care. If you need care be<strong>for</strong>e the new PCP is effective,<br />

call the PCP on your child’s current ID card.<br />

Here are some reasons why you may want to change your child’s PCP:<br />

• You are not happy with the current PCP’s care<br />

• You need a different kind of doctor to take care of your child<br />

• Your child’s PCP is no longer near you because you have moved<br />

• Your child’s PCP is no longer a part of <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> or <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate<br />

Are There Any Reasons Why My Request to<br />

Change a PCP May Be Denied?<br />

Here are some reasons why your child may not be able to have the PCP you chose:<br />

• The PCP you picked cannot see new patients<br />

• The PCP you picked is no longer a part of <strong>Parkl<strong>and</strong></strong> <strong>KIDSfirst</strong> or <strong>Parkl<strong>and</strong></strong> <strong>CHIP</strong> Perinate<br />

Can a PCP request that I/ my child be changed to another<br />

PCP <strong>for</strong> noncompliance?<br />

Here are some reasons why your child’s PCP may ask that you pick a new PCP:<br />

• Your child frequently misses appointments <strong>and</strong> you have not called to let the PCP know<br />

• You are not following your child’s PCP’s advice<br />
