The Basics: Eligibility, Whom You Can Cover,<br />

Enrolling and Costs<br />

Eligibility for Benefits<br />

To be eligible for coverage under our benefit plans, you must be:<br />

• A regular full-time employee or a buffer employee or regularly scheduled<br />

to work at least 30 hours per week.<br />

To cover your dependents, they must be enrolled in the same medical, dental<br />

and vision plan you choose for yourself. But you don’t need to cover the same<br />

dependents for medical, dental and vision. For example, you may choose to cover<br />

yourself, your spouse and children for medical but only yourself and your children<br />

for dental and vision.<br />

When you enroll your dependents for the first time, you must provide proof that they<br />

are eligible for coverage. See page 26 for more details.<br />

When You Can Enroll<br />

You can enroll in or make changes to your benefits during these enrollment windows:<br />

During Annual <strong>Enrollment</strong> each fall. This is the annual time you can change your benefit<br />

plans and add or drop covered dependents.<br />

When your life changes. Certain events such as getting married or having a child<br />

allow you to make changes to your benefits. Make allowable changes within 30 days<br />

of the event. If you miss this deadline, you can’t make changes until the next Annual<br />

<strong>Enrollment</strong> period. For more information about other actions to consider during a life<br />

As a new employee. If you are in PBMS, Mail Services, Level One or certain Customer<br />

Ops, you are eligible for benefits on the first of the month after you reach 90 days<br />

of service. For all other business units, you are eligible on the first of the month<br />

following your date of hire. You must enroll before the 15th of the month in which<br />

you became eligible.


How You and <strong>Pitney</strong> <strong>Bowes</strong> Share Costs for Benefits<br />

<strong>Pitney</strong> <strong>Bowes</strong> provides some benefits at no cost to you. For others, you and <strong>Pitney</strong><br />

<strong>Bowes</strong> share the cost. And there are some plans where you pay the entire cost. Your<br />

cost will be deducted from your paycheck twice a month during the calendar year.<br />

Covering a working spouse or domestic partner? Enrolling a working spouse or<br />

domestic partner in your <strong>Pitney</strong> <strong>Bowes</strong> medical and dental coverage when they can<br />

obtain coverage through their own employer means you’ll pay more annually—$1,500<br />

for medical coverage and $150 for dental coverage. This doesn’t apply if you both<br />

work for <strong>Pitney</strong> <strong>Bowes</strong>.<br />

Pay less for medical coverage. Complete the short, online health assessment through<br />

your medical plan by November 30, 2012, and pay less for medical coverage in 2013—<br />

$100 (you) or $200 (you + your spouse/domestic partner). This health assessment<br />

credit is part of the PB Healthy Rewards Program.<br />

Employees who are first eligible for benefits on or after January 1, 2013, are not<br />

eligible for the 2013 health assessment credit, but you still need to complete the<br />

assessment through your medical plan after you enroll in coverage to earn incentives<br />

in 2013.<br />

spouse/domestic partner). The program includes three steps. You access each step<br />

through your medical plan:<br />

1. Take the health assessment by November 30, 2012.<br />

2. Complete the biometric screening between January and March 2013 (when possible).<br />

3. Participate in healthy programs and activities April through October 2013.<br />

Medical Plan Options<br />

All of our medical plans provide you with preventive care and comprehensive<br />

coverage to protect you in case of a serious accident or illness. All plans include<br />

prescription drug and behavioral health coverage. The key differences are how you<br />

pay for services during the year and the plan administrator. There are three types of<br />

medical plans available.<br />

HMO Plan You pay a higher fixed cost through paycheck deductions regardless of<br />

how much care you use. Coverage is limited to inside the HMO network (except in<br />

certain emergencies). The HMO option is available only in certain states.* You can<br />

choose the Health Care Account to help you pay for health care expenses.<br />

*If you live in certain parts of CA, CO, GA, HI, OR, WA or the Mid-Atlantic area, you have an HMO option with Kaiser and/or<br />

Group Health Cooperative.<br />

A Health Account can be used to cover eligible expenses, such as deductibles,<br />

coinsurance and copayments with tax-free dollars. Your medical plan choice<br />

determines the Health Account(s) that can be paired with it. How the accounts<br />

Choosing the Health Fund Plan?<br />

• Health Reimbursement Account (HRA) – <strong>Pitney</strong> <strong>Bowes</strong> contributes to the account<br />

and funds are available the first day your benefits become effective (new hires)<br />

or January 1 (current employees). This account is linked directly to your medical<br />

plan, and your costs are automatically deducted from the account when your<br />

claim is processed until you reach a $0 balance. You can increase your HRA<br />

balance by earning the full PB Healthy Rewards incentive.<br />

• Health Savings Account (HSA) – Both you and <strong>Pitney</strong> <strong>Bowes</strong> contribute to the<br />

STD Plan depend on which business unit you are in: Level One, Call Center, PBMS<br />

and PBGS employees are not eligible for STD benefits during your first year of service;<br />

PBI employees are eligible for STD payments up to 26 days, if approved, during your<br />

first year of service. After 21 weeks, this coverage either ends or your long-term<br />

disability (LTD) coverage begins.<br />

Critical Illness Insurance<br />

Critical Illness Insurance through MetLife complements your medical and disability<br />

income coverage. If you or a covered family member becomes critically ill, you receive<br />

a lump-sum benefit that you can use for anything—medical expenses, household<br />

expenses, rent/mortgage payments, child care and more. You can elect Critical Illness<br />

Insurance if you’re eligible for benefits, but you do not have to be in a <strong>Pitney</strong> <strong>Bowes</strong><br />

medical plan. There’s no waiting period for coverage to begin.<br />

Coverage is optional and you pay the full cost. You can purchase coverage for yourself,<br />

your spouse or domestic partner and your child(ren) at a level of $10,000 each.<br />
