Supplemental Accidental Death and Dismemberment (AD&D) Insurance<br />
Accidental Death and Dismemberment (AD&D) Insurance protects your family financially<br />
if you or a covered dependent dies or loses a limb, eye or hearing in an accident.<br />
For you: 1–5 times your pay; up to a maximum of $2 million<br />
For your spouse/domestic partner: Up to $250,000 in $10,000 increments<br />
For your children: $10,000 or $20,000<br />
Choosing Supplemental Life Insurance or AD&D?<br />
You may be required to submit proof of good health before your new coverage<br />
becomes effective. This process can involve urine and blood tests (screenings),<br />
along with a physical exam, to be scheduled by a physician.<br />
Supplemental Long-Term Disability<br />
Increase the 50% income replacement you already receive through basic LTD<br />
to 66 2 /3% of your monthly pay. You can choose to pay for coverage with pretax<br />
or after-tax dollars. Supplemental LTD is not available to Buffer employees.<br />
Critical Illness Insurance<br />
Critical Illness Insurance through MetLife complements your medical and disability<br />
income coverage. If you or a covered family member becomes critically ill, you receive<br />
a lump-sum benefit that you can use for anything—medical expenses, household<br />
expenses, rent/mortgage payments, child care and more. You can elect Critical Illness<br />
Insurance if you’re eligible for benefits, but you do not have to be in a <strong>Pitney</strong> <strong>Bowes</strong><br />
medical plan. There’s no waiting period for coverage to begin.<br />
Coverage is optional and you pay the full cost. You can purchase coverage for yourself,<br />
your spouse or domestic partner and your child(ren) at a level of $10,000 each.<br />

Benefits are paid to each covered person as a percentage of the amount of coverage<br />
you elect up to the $30,000 maximum. You receive payment for the first incident of an<br />
illness. And for certain conditions (heart attack, coronary artery bypass graft surgery<br />
and cancer), if you face the illness again, you can receive a 50% reoccurrence benefit.<br />
There are also certain conditions such as cystic fibrosis and multiple sclerosis for<br />
which each covered person can receive a one-time benefit of 25%. And, if you receive<br />
a major organ transplant, you will receive 100% of your elected benefit amount.<br />
Critical Illness Insurance benefits are subject to lifetime maximum—$30,000 total<br />
benefit amount plus the $10,000 major organ transplant benefit. Once you reach the<br />
lifetime maximum, your coverage will end.<br />
Guaranteed issue<br />
No proof of good health is required when you purchase coverage for 2013.<br />
