How to Enroll and Important Reminders Follow the steps below to compare your options, and choose the benefits that best fit your needs and your budget. Do Your Homework Understand and compare your options. These tools and resources can help: • Health Plan Comparison Charts on the Your Benefits Resources website at resources.hewitt.com/ pitneybowes. • UHC Cost Estimator Tool at www.myuhc.com/groups/Pitney. You can use the tool even if you don’t choose the UHC medical plan. • Health Plan Websites and Member Services. Go to www.pbprojectliving.com > U.S. Benefits > Contacts & Resources for a full list of plan providers and contact information. • Pitney Bowes Benefits Center: Representatives are available to answer your questions from 8:30 a.m. to 6:30 p.m. EST, Monday– Friday by calling 1-888-5PB-FLEX (1-888-572-3539). Choose Your Benefits Enroll for the first time or make changes to your current coverage. • Online at Your Benefits Resources at resources.hewitt.com/pitneybowes. Log in with your user ID and password. Forgot your user ID or password? Click “I forgot my user ID” and/or “I forgot my password” on the Login page, or call the Benefits Center to create a new user ID or password. • Over the phone by calling 1-888-5PB-FLEX (1-888-572-3539). Enrolling for the first time? Have this information handy: • Your Social Security number • Your birth date • Your home ZIP code Enrolling new dependents? Have this information handy: • Name(s) • Birth date(s) • Social Security number(s) (you won’t be able to enter any dependent information without these) 26
Don’t Forget … Enroll by the deadline. If you don’t take action, you’ll automatically be enrolled in “default” coverage—and be responsible for the costs—that may not meet your needs and budget. • If you are already enrolled in Pitney Bowes benefits coverage and you do not take action during Annual Enrollment, you will remain in your current coverage for medical, dental, vision, optional life insurance, Health Care Account (HCA), Dependent Care Account (DCA), Group Legal and Flex Days for 2013. • If you are newly eligible for benefits and do not enroll by the deadline, you will automatically be enrolled in the Cigna Health Fund Plan with HRA, Delta Dental Preventive Plan, Basic Life Insurance and Basic Long-Term Disability. You will not participate in any other benefit programs that require your enrollment: Health Care Account (HCA) or Dependent Care Account (DCA), Supplemental Life Insurance, Critical Illness Insurance, HealthAllies Discounts, Voluntary Vision, Accidental Death & Dismemberment (AD&D), Group Legal, or Flex Days. Verify your dependents. When enrolling, you must verify that your dependents are eligible for coverage. If you don’t provide proof when requested, coverage for your dependents will not become effective, and any reimbursement claims on behalf of that dependent will be denied. If you need additional information, please call the Benefits Center at 1-888-5PB-FLEX. To verify your dependents, you’ll need: • A Social Security number for each dependent • The Social Security number for a newborn child within 60 days of birth • Certification that your domestic partnership meets the eligibility requirements Update beneficiaries. Ensure your beneficiary designations for your insurance plans are up to date. Visit Your Benefits Resources at resources. hewitt.com/pitneybowes or call Benefits Center at 1-888-5PB-FLEX. Current employees: Take the health assessment through your medical plan by November 30, 2012 to lower your medical paycheck deductions and complete one of the steps needed to earn more incentives under the PB Healthy Rewards Program in 2013. Check Your Confirmation Statement Watch for your confirmation statement arriving in the mail or when it becomes available online. It explains your coverage for the next year. Review it carefully and report any errors to the Benefits Center at 1-888-5PB-FLEX. 27
