Dental Coverage Dental coverage helps you maintain good oral health, and it can help offset costs if you or a family member needs dental or orthodontia services in 2013. FREE Delta Dental Preventive Plan is free to most employees Delta Dental Preventive Plan (no cost to you) Free to most employees, except PBGS employees who do not receive Flex$, this plan covers your routine exams, cleanings, fluoride applications for children and full-mouth or bitewing X-rays. If you need additional services during the year, you are responsible for those costs. But you can pay for them through your Health Savings Account (HSA) or Health Care Account (HCA). Delta Dental Premium Plan and Aetna Dental DMO (extra coverage if you choose) You can purchase additional coverage through one of these plans if you need it: Choice of Doctors Annual Deductible Calendar Year Maximum (does not apply to preventive or orthodontic care) Delta Dental Premium Receive care in- and out-of-network, but to make the most of your coverage, choose a Delta Dental PPO or Premier dentist. You pay $100 on basic and major care Aetna Dental DMO Use an Aetna provider. You choose a primary care dentist to coordinate your care and refer you to a specialist if needed. Vision Coverage Vision coverage can help offset the costs of eyeglasses and contact lenses. All our medical plans provide a free, basic annual eye exam performed by an in-network doctor. For other vision services, depending on the vision plan you enroll in, you receive either a discount on the services or you receive an allowance paid by the vision plan. EyeMed Discount Program (no cost to you) Get discounts on eye exams, glasses, contact lenses and more from EyeMed Select providers. You have access to this discount program even if you are not enrolled in medical coverage. • You receive a 40% discount on the retail cost of frames. • Copays start at $50 for lenses, and you are eligible for additional discounts at all EyeMed locations. • You receive a 15% discount on the retail cost of conventional contact lenses (disposable lenses not included). • Each person covered under the plan has unlimited use of the frame and lens (or contact lens) benefits. Voluntary Vision Option (extra coverage if you choose) This option offers a better benefit than the EyeMed Discount Program if you need vision correction, a pair of glasses (frame and lenses) or contact lenses. You’ll get an allowance for frames and will be responsible only for the copay amount for lenses. • You receive a $135 frame allowance when buying eyeglasses from an EyeMed Select provider plus 20% discount on any balance over the allowance. • $10 copay for single, bifocal and trifocal lenses and you are eligible for additional discounts at all EyeMed locations. • Each person covered under the plan receives frame and lens (or contact lens) benefits once per calendar year. • $70 copay for standard and premium progressive lens, and for premium progressive lenses you receive a 20% discount and a $120 allowance. • $135 allowance for contact lenses. • You get a 40% discount on additional complete pairs of glasses (frames and lenses) and a 15% discount on conventional contact lenses (not including disposable lenses) once your maximum allowance has been reached.
