BRIEF RESUME<br />
Wan Aminnudin Wan Ismail<br />
He has been in the health and safety field for the past 12 years; involved with safety audits, machinery<br />
safety, equipment maintenance, industrial hygiene and accident investigation. He is currently an active<br />
team member <strong>of</strong> Forensic Engineering Division.<br />
Academically, he has a bachelor degree in manufacturing engineering from University <strong>of</strong> Birmingham,<br />
UK (1999) before joining the Department <strong>of</strong> Occupational Safety and Health in 2000. He was then been<br />
given the opportunity to pursue his master’s in the field <strong>of</strong> forensic engineering, University <strong>of</strong> Teeside,<br />
UK (2009).<br />
Ir Dr Mohd Fairuz Ab Rahman<br />
Fairuz joined DOSH in July 2003 as a design engineer, whose main responsibility is to issue approval for<br />
installation <strong>of</strong> elevator and escalator. He was on study leave from March 2009 until January 2012, and<br />
rejoined DOSH as a forensic engineer. Prior to joining DOSH, Fairuz held a position as a mechanical<br />
engineer with HeveaBoard Berhad (Dec 2000 – May 2002) and a technical planning engineer with Gas<br />
<strong>Malaysia</strong> Berhad (Jun 2002 – July 2003).<br />
Fairuz has a bachelor degree in mechanical engineering from the Sheffield University, UK (2000), a master<br />
<strong>of</strong> science in integrated construction project management from the Universiti Teknologi MARA, <strong>Malaysia</strong><br />
(2006) and a doctor <strong>of</strong> philosophy in mechanical engineering from RMIT University, Australia (2012).<br />
Fairuz studied fatigue crack growth phenomenon in aluminium alloys and developed a new validated<br />
crack growth predictive model for assessing fatigue fracture surfaces. He has published two journal<br />
papers and three conference papers.<br />
Fairuz is registered as a pr<strong>of</strong>essional engineer with the Board <strong>of</strong> <strong>Engineers</strong> <strong>Malaysia</strong> and is a corporate<br />
member <strong>of</strong> the Institute <strong>of</strong> <strong>Engineers</strong>, <strong>Malaysia</strong>.<br />

TOPICS<br />
A. Forensic Engineering Investigation<br />
This is an introductory topic which will highlight the essence and principle <strong>of</strong> forensic engineering, and the<br />
essential parts <strong>of</strong> an investigation. Some tools in investigation will also be introduced.<br />
B. Incident Scene Investigation and Case Study<br />
This slot will discuss about the various issues that need to be considered during an incident scene<br />
investigation, including tips on some issues that you may encounter.<br />
A few case studies will also be presented and discussed. This will encompasses real life industrial accident<br />
cases which happened throughout <strong>Malaysia</strong>, and investigated by our team.<br />
