My name is (introduce self) and this is (introduce note-taker). Thank you for agreeing to<br />

participate in today’s interview regarding the <strong>CIS</strong> <strong>of</strong> [insert name <strong>of</strong> local <strong>CIS</strong> program].<br />

We greatly appreciate you taking time out <strong>of</strong> your busy schedule to assist with the<br />

evaluation <strong>of</strong> <strong>CIS</strong> <strong>of</strong> <strong>Texas</strong> that ICF International is conducting for the <strong>Texas</strong> Education<br />

Agency. You were selected to participate in an interview because your perspective will<br />

help us to better understand issues relevant to this evaluation. We are conducting case<br />

studies with five local programs across <strong>Texas</strong> to gather information about the<br />

implementation <strong>of</strong> <strong>CIS</strong>, relationships within <strong>CIS</strong>, training and support, best practices, and<br />

much more. Your participation in this interview is completely voluntary. .Please feel free<br />

to be open and candid in your responses to our questions, as we will keep this<br />

information strictly confidential. Only general themes will be conveyed in our final report<br />

(your name will not be linked to anything that you say).<br />

With your permission, we would like to record the audio <strong>of</strong> this interview so that we can<br />

transcribe the conversation for accuracy in the analysis and interpretation <strong>of</strong> your<br />

comments along with comments <strong>of</strong> other staff. No one from the State Office or <strong>TEA</strong> will<br />

have access to this audio recording. Upon transcription <strong>of</strong> these recordings as appropriate<br />

to the evaluation, we will destroy the recordings themselves, maintaining only written<br />

records. Only de-identified transcripts <strong>of</strong> recordings will be the property <strong>of</strong> <strong>TEA</strong> at any<br />

time during or after the contract period.<br />
