Free flaps‐ the DIEP • What is it • Deep Inferior Epigastric Artery Perforator flap • Similar concept to free TRAM • Involves dissection of the artery from the muscle and a perforating vessel which supplies the skin and fat • Preserves muscle and fascia so strength and integrity are maintained • Technically longer surgery, but less invasive • Possible risk of fat necrosis • Very appealing to patients, newer technique, and a useful form of reconstruction
