2012/13.
Report from CCG Commissioning Sub Group
including Transformation Bids
Dr Avi Prasad reported on the transformation bids that
had been discussed at the CCG Commissioning Sub
Group (Paper L). In August the CCG was made aware
of a further £1.5m to be used non-recurrently and rapid
work was done to come up with ideas which then went
through a shortlisting process. Most of the bids
submitted did support the mission and vision of the
organisation.
UCC bounce back scheme
This scheme would require putting Systmone into all
practices so that patients attending the UCC could be
registered with a practice and be given a GP
appointment there and then.
Respiratory Disease Outcomes
This would improve care in the community and prevent
admissions. The virtual ward is not part of this.
Community Dermatology
This requires the up-skill of GPs to diagnose and treat
and provide an OP service in the community.
Audio and Visual Communication
Information screens would give patients information
about health services and signpost them to the most
appropriate part of the NHS system.
Twinning
This would partner or cluster practices to share learning
and best practice.
Protected Learning Time
City wide and locality protected learning time for
practices is pivotal to deliver organisation wide changes
and developments.
24 hour blood pressure monitors
This is part of NICE guidelines and practices would be
assisted to fulfil the criteria.
