Can I switch my baby’s health plan<br />

Your baby will be assigned to the same health plan that you are enrolled with for at least 90 days after the date of<br />

birth. You can ask to change health plans before the 90 days if both health plans agree to the change. You will<br />

need to contact HHSC to keep your baby on Medicaid before the end of the 90 days.<br />

If your baby is in the hospital, you will not be able to change health plans until your baby is discharged.<br />

What is Case Management for Children and Pregnant Women (CPW)<br />

CPW is a program offered by the State. It provides services to children with a health condition or risk, birth<br />

through 20 years of age and high-risk pregnant women of all ages.<br />

What type of services would my child or I receive<br />

Case management is a Medicaid service. They can help you or your child with:<br />

- Getting medical services,<br />

- Family problems,<br />

- School issues,<br />

- Money concerns,<br />

- Finding help near where you live, and<br />

- Equipment and supplies.<br />

Who do I call if I have special health care needs and need someone to help me<br />

You can have your health care from a specialist if you have special health care needs. If you have special health<br />

care needs and need someone to help you, please call <strong>Parkland</strong> HEALTHfirst. Call 1-888-672-2277 and ask for a<br />

case manager.<br />

What do I do if I have to move<br />

Report your new address as soon as possible to the local HHSC Eligibility Office and <strong>Parkland</strong> HEALTHfirst<br />

<strong>Member</strong> Services at 1-888-672-2277. You must call <strong>Parkland</strong> HEALTHfirst before getting any services in your<br />

new area unless it is an emergency. You will continue to get care through <strong>Parkland</strong> HEALTHfirst until the<br />

address is changed unless you have moved out of the service area.<br />
