Credit is granted only once. The repeat grade will replace<br />

the previous grade in the GPA calculation. However, the<br />

previous grade (F) will still appear on the transcript.<br />

<strong>STUDENT</strong> PROGRESS ASSESSMENT<br />

In the sophomore and/or junior years, students are<br />

carefully evaluated by several instructors in their division<br />

and separately by a member <strong>of</strong> the Liberal <strong>Art</strong>s faculty.<br />

The students’ work is reviewed, problems or weaknesses<br />

discussed, and students are advised on their progress. This<br />

provides an opportunity for the faculty to become better<br />

acquainted with the students and their work.<br />

All students must complete the Student Progress<br />

Assessment as a requirement for graduation. Students<br />

who change their major area <strong>of</strong> study must complete a<br />

Student Progress Assessment for the division in which they<br />

expect their degree. Most division assessments fulfill the<br />

requirement for that division only; not all divisions allow<br />

substitution <strong>of</strong> another division’s assessment for their<br />

own. The Student Progress Assessment is conducted in<br />

Spring Semester on dates indicated on the college calendar.<br />

Students must add the assessment to their schedules<br />

according to their major and class rank at the time <strong>of</strong><br />

registration. The following chart indicates the students in<br />

each division who must register for an assessment:<br />

Students who are not sure whether they should be reviewed<br />

should contact the Advising <strong>of</strong>fice or their division dean.<br />

Details concerning specific days, appointment times, and<br />

assessment procedures will be posted around campus prior<br />

to the Student Progress Assessment.<br />

TRANSCRIPT REQUESTS<br />

Transcripts can be requested in person, by mail, or online.<br />

Approximately 5–7 days must be allowed to process<br />

transcript requests.<br />

Students requesting transcript service should plan in<br />

advance. Last-minute requests are difficult to process quickly<br />

during certain periods <strong>of</strong> the academic year. Transcript<br />

requests must include the student’s full name, maiden name,<br />

signature, Social Security number, date <strong>of</strong> birth, current<br />

address, years <strong>of</strong> attendance, number <strong>of</strong> copies desired,<br />

and the complete names and addresses <strong>of</strong> individuals or<br />

organizations that are to receive the transcripts. There is a<br />

$5 charge for each <strong>of</strong>ficial transcript requested. An additional<br />

fee <strong>of</strong> $10 will be charged for rush service. To comply with<br />

the Family Educational Rights and Privacy Act <strong>of</strong> 1974, the<br />

college must have the student’s written consent to release<br />

educational records.<br />

Transcript request forms can be obtained from the One-Stop<br />

Student Services <strong>of</strong>fice or online through the IQ Web system.<br />

Current students may request transcripts online via the IQ<br />

Web system. For more information, go to the “Transcript<br />

and Enrollment Verification” page under One-Stop Student<br />

Services at www.ccad.edu.
