Prenatal ultrasound diagnosis of s<strong>in</strong>gle umbilical artery (SUA) and pregnancy outcomes<br />

Although growth restriction was cl<strong>in</strong>ically and<br />

sonographically suspected <strong>in</strong> one case, the birth weight was<br />

<strong>in</strong> excess of 2500g. Growth restriction is reported to occur <strong>in</strong><br />

association with SUA and was detected <strong>in</strong> 10.2% <strong>in</strong> one study 9 .<br />

One patient demonstrated elevated cord Doppler values but<br />

this pregnancy was complicated by hydramnios. Doppler<br />

read<strong>in</strong>gs <strong>in</strong> pregnancies with SUA are generally towards the<br />

lower end of the normal range, attributed to the larger arterial<br />

diameter which may be associated with decreased flow<br />

resistance 10 . Doppler abnormalities have been reported to occur<br />

<strong>in</strong> 30% of fetuses with SUA. Abnormal Doppler read<strong>in</strong>gs <strong>in</strong><br />

SUA are highly associated with growth restriction, complex<br />

structural mal<strong>for</strong>mations or an abnormal karyotype 11 .<br />

A suggested protocol <strong>for</strong> the management of prenatally<br />

diagnosed SUA on ultrasound <strong>in</strong>cludes a detailed<br />

sonographic evaluation with fetal echocardiography<br />

possibly at a tertiary fetal ultrasound or cardiac unit. The<br />

f<strong>in</strong>d<strong>in</strong>g of a second pert<strong>in</strong>ent sonographic abnormality is<br />

an <strong>in</strong>dication <strong>for</strong> karyotype determ<strong>in</strong>ation. Cl<strong>in</strong>ical growth<br />

monitor<strong>in</strong>g <strong>for</strong> the rema<strong>in</strong>der of the pregnancy appears<br />

<strong>in</strong>dicated, augmented by Doppler velocimetry where there<br />

is cl<strong>in</strong>ical concern. Counsell<strong>in</strong>g of the parents should<br />

<strong>in</strong>clude a description of the recognized associations and<br />

impact on fetal/per<strong>in</strong>atal mortality.<br />
