<strong>North</strong> <strong>Yarra</strong> <strong>Community</strong> Health aims to provide high quality and responsive<br />
programs and services to all members of our community.<br />
We work with people to overcome their experiences of<br />
disadvantage, discrimination and disempowerment.<br />
We respect people’s choices and support them to make informed decisions to promote their<br />
health and well-being.<br />

President’s and CEO’s <strong>Report</strong><br />
During the 2003-04 year, NYCH celebrated our 10-year<br />
anniversary. In 1993, the <strong>Community</strong> Health Centres of<br />
Carlton, Collingwood and Fitzroy amalgamated to form one<br />
organisation, and the inaugural Board of Management<br />
meeting was held on September 7th 1993.<br />

Health Project in partnership with St Vincent’s Hospital<br />
• Numerous achievements by the participants of the two<br />
<strong>Community</strong> Empowerment Projects with which we have<br />
been involved, including the much publicised Laughing<br />
Group, and the Campaign against Centrelink Closure<br />
• Launch of the Afghani/ Iranian Women’s Group Mural,<br />
which you can see on the front cover<br />
In conclusion, we would like to acknowledge our funders,<br />
the Victorian Department of Human Services, the Commonwealth<br />
Department of Health and Ageing, Arts Victoria and the Australia<br />
Council, the Reichstein Foundation, the City of <strong>Yarra</strong> and the<br />
many individuals and businesses that have supported us with<br />
donations and in-kind contributions throughout the year.<br />
A special word of thanks to our volunteers, who add so<br />
much value to the work that we do. They take exercise groups,<br />
drive the bus, facilitate social groups, help with administrative<br />
tasks and support the staff in many different ways.<br />
We are extremely grateful to the many people who give<br />
so generously of their time.<br />

We would like to take this opportunity to acknowledge the<br />
work of those Board Members, and all the other people who<br />
have served on the Board over the past 10 years. In particular,<br />
we wish to acknowledge the significant contribution of Edith<br />
Morgan, who died on May 10th 2004. Edith was a great<br />
supporter and maintained an active interest in NYCH. She will<br />
be greatly missed by both Board Members and Staff.<br />
A lot has changed since 1993. In the intervening ten years we<br />
have introduced numerous new programs and services,<br />
including: Occupational Therapy, Speech Pathology, Nutrition,<br />
Youth Work, Next Door - Primary Health Facility for people<br />
who inject drugs, Hospital Admission Risk Programs, Health<br />
Promotion in Public Housing as well as numerous programs<br />
for refugees and holders of Temporary Protection Visas,<br />
people who are homeless or living in insecure housing,<br />
people with psychiatric illnesses, and older residents of High<br />
Rise Estates.<br />
Highlights of the 2003 - 04 year<br />
While the celebration of our 10th anniversary was a major<br />
highlight of the 2003/04 year, other highlights have included:<br />
• Inception of "What’s the Deal" an early intervention<br />
Problem Gambling Project with young people on the<br />
Collingwood Housing Estate<br />
• Funding from the Australia Council and Arts Victoria for two<br />
theatre productions on the Fitzroy Housing Estate<br />
• Publication and launch of "Eating Well Today - For<br />
Tomorrow", our paediatric needs assessment into the eating<br />
habits of children aged 0-5<br />
• Funding from the National Illicit Drug Strategy for the<br />
employment of a Psychiatric Nurse to work with clients at<br />
the Next Door Primary Care Facility, and funding as part of<br />
the Local Drug Strategy for a case manager with this high<br />
needs client group<br />

We also thank the many other agencies with whom we<br />
have worked collaboratively during the year, including the<br />
member agencies of the Primary Care Partnership,<br />
Neighbourhood Houses and other <strong>Community</strong> Health<br />
Centres, Collingwood <strong>Community</strong> Information Centre,<br />
Melbourne City Mission, Collingwood College, the Reach<br />
Foundation, Jesuit Social Services, Brotherhood of<br />
St Laurence, Homeground, and our local hospitals.<br />
Our joint efforts achieve so much more than any of us<br />
could achieve individually.<br />
Board members also give of their time, and it is the Board<br />
that determines the overall policy of the organisation. During<br />
the year, elections were held, and as a result, we are pleased<br />
to welcome Felicity Pantelidis who will join the board of<br />
Management in October 2004.<br />
Two of the current Board members, Sandra Hills and Lisa<br />
Hurburgh have indicated that they will not be seeking further<br />
terms, and accordingly, their term of office concludes at the<br />
2004 <strong>Annual</strong> General Meeting. On behalf of the other Board<br />
members and the staff, we would like to thank Sandra and<br />
Lisa for their contribution to NYCH over the past three years.<br />
Most importantly, we want to acknowledge the work of the<br />
staff. It is their drive, initiative and enthusiasm which,<br />
together with the input and feedback from our clients,<br />
ensures that we continue to deliver the sort of services that<br />
the community needs and wants.<br />

The <strong>Community</strong> Liaison Committee has been meeting every three months since 1994, and<br />
it provides an opportunity for our clients to receive information, give us feedback, and raise<br />
matters of concern. Most importantly, it strengthens community cohesion and enables the<br />

the <strong>Community</strong> Empowerment Project, and there is no doubt that supporting<br />
parents is the best and most effective way to ensure that children are also<br />
supported and nurtured. Other programs that support children include our<br />
popular Hanen groups “It takes two to talk”. These therapy groups assist parents<br />
to help their child develop and improve language skills. Michael and Anna Black<br />
(shown bottom left with Thomas) have been attending one of the Hanen groups.<br />

our program for people living in the community with psychiatric<br />
disabilities. As part of that celebration, participants collaborated in the<br />
production of a book that records their journey to recovery. The striking<br />
cover was designed by Yurdanur Bozkurt.

We live in a multicultural society, and our<br />
local community is particularly diverse.<br />
To underscore this multiculturalism, the<br />
Afghani/ Iranian Women’s Group wanted to<br />
go to the cricket last year, and thoroughly<br />
enjoyed their day out.<br />
The African Women’s Group has been meeting at<br />
the Collingwood Neighbourhood House with our<br />
midwives and our Family And Reproductive Rights<br />
Program Worker to provide mutual support and to<br />
increase their knowledge of health services<br />
available in the community.<br />

improve fitness, while making important social connections. The Vietnamese Tai<br />
Chi group meets at the Collingwood Centre, the Walking Group meets at Carlton<br />
(and another walking group meets at Collingwood) while the Gentle Exercise to<br />

Access is a right, not a privilege<br />
The mural depicted has been painted on the entrance to the<br />
underground car park on the Collingwood High Rise estate. The<br />
artists, Eugene Lovett, Gary Smith, Mary Hassall and Denise<br />
Lovett are part of the indigenous community, which has been living<br />
and meeting in Collingwood and Fitzroy for many years. Working as<br />
the BEEM artists, they have painted a number of other murals in<br />
the local area, and last year they donated the proceeds from the<br />
sale of one of their paintings to the Collingwood Youth Space.

Prevention is better than cure<br />
These happy vegetables, Basil Natoli (Capsicum) and Peta Christensen<br />
(Green Pea) from DHS joined with Lisa Harris (Carrot) and Lina La Guardia<br />
(Tomato) to spread the healthy eating message at the launch of the Atherton<br />
Gardens Fresh Food Market. Over 90 people completed the Healthy Eating<br />
Quiz, and Loan Nguyen was the winner of a delicious hamper of food.<br />
A number of "Eat Well, Be Well, Live Well" Expos have been held throughout<br />
the year, and more are planned for next year. For further information,<br />

of programs and interventions to our clients. This mural<br />
was painted by the participants in our Art Therapy group,<br />
which is facilitated by Art Therapy student, Celia.<br />
It is on display in the window of our shop front at<br />

Our philosophy is to encourage people to take better control over their<br />
health, and we can help you to do that, either individually or through<br />
some of our group programs. The Collingwood Estate Gym is a<br />
valuable community resource, and it’s open every weeknight from 5.00<br />
pm. Leon, Ricardo and Paul regularly use the gym to improve their<br />
fitness, while Christine and Jennifer attend our Women Living Stronger<br />
(Weights Training) class, which runs weekly. Jennifer says "I really like<br />
the relaxed atmosphere of the group - it makes it fun and enjoyable!"

The Collingwood Youth Space opened in 2003, and has recently<br />
celebrated its first birthday. Young people come together for a<br />
range of activities, which includes some excursions.<br />

The Collingwood Laughing Group was one of the outcomes from<br />
the <strong>Community</strong> Empowerment Project. They epitomise the old<br />
saying about "Laughter being the best medicine" and are part of<br />
a worldwide movement. Judging by the graffiti, on the rubbish<br />
bin, they are certainly making a difference!<br />

The Counselling and Casework Team provides<br />
counselling, support, information and advocacy<br />
to individuals, couples and families across our<br />
centres.<br />
The Team consists of:<br />
• Family Counsellor: Provides counselling,<br />
casework and groups for families living in<br />
Carlton and Parkville.<br />
• Refugee Support Worker: Provides a<br />
mix of casework, community development and group<br />
support to refugees, including people on Temporary<br />
Protection Visas and asylum seekers.<br />
• Family And Reproductive Rights Education Program<br />
(FARREP):<br />
Has a focus on community development, health promotion<br />
and casework predominately with people from the Horn of<br />
Africa countries.<br />
• Youth Worker: Undertakes individual support, groups and<br />
facilitates a drop-in space for young people aged 12 -18<br />
living in Collingwood.<br />
• Older Persons’ High Rise Support Program:<br />
Provides casework, support, groups and drop-in for<br />
people living at the 530 Lygon Street flats.<br />

• Drug and Alcohol Counsellor: Provides counselling to people<br />
in relation to problematic alcohol and other drug use.<br />
• General Counselling and Casework: Provides a<br />
combination of short, medium and long term counselling,<br />
casework and advocacy. Typical reasons for attendance include<br />
family violence, immigration, relationship difficulties,<br />
depression, stress, housing and a range of other issues.<br />
The year has seen some exciting new programs developed,<br />
including What’s the deal an early intervention art therapy<br />
project focusing on the impact of gambling on young people,<br />
and the Halal Food Access Project, a new group developed<br />
to increase food access and reduce social isolation for the<br />
Fitzroy Muslim population.<br />

life, rich and poor, old<br />
and young, and NYCH<br />
practitioners are<br />
experienced and able to<br />
provide treatment and<br />
support options to meet<br />
individual needs.<br />
In particular, the <strong>Yarra</strong><br />
<strong>Community</strong> Support<br />
Team, our Psychiatric<br />
Disability Rehabilitation and Support Service, works with<br />
people on a wide range of issues to promote recovery and<br />
enhance quality of life. The Team operates on a home-based<br />
outreach model and can bring services to people within their<br />
home or the community.<br />
The Team was proud to celebrate 10 years of service delivery<br />
in September, and to reflect on the growth and achievements<br />
over the decade since a humble beginning in 1993. The service<br />
now has the capacity to support 50 people at any one time<br />
across Carlton and the City of <strong>Yarra</strong> and is an integral part of<br />
mental health services in the area. To mark the occasion, past<br />
and present participants put together a booklet of poetry, prose<br />
and artwork. This beautiful collection of material was launched<br />
at a special 10th birthday celebration and has been distributed<br />
to organisations and individuals across the City of <strong>Yarra</strong>.<br />
The year has seen the consolidation of the intensive support<br />
program for people who are homeless or at risk of<br />
homelessness and it has been inspirational to see the<br />
difference stable housing with support can make to people<br />
living with a mental illness.<br />
The Team continues to have a strong and important role in<br />
advocating the needs of people with a mental illness to<br />
ensure that services and systems are appropriate and<br />
accessible. To this end positive relationships have been<br />
developed with the Consumer Consultant at Inner East Mental<br />
Health Service and Victorian Mental Illness Awareness Council.<br />
YCS was again successful in securing a <strong>Community</strong> Grant<br />
from the City of <strong>Yarra</strong> to continue the popular Pathways for<br />
Women program. This program is run in conjunction with<br />
local <strong>Yarra</strong> services and has a focus on assisting women to<br />
develop a range of skills, raise awareness of general health<br />
issues and provide access to other NYCH services.<br />
Drug Safety Services encompass a very broad range of programs,<br />
but all of them have a Harm Reduction philosophy, and seek to<br />
reduce the harm associated with illicit drug use to users<br />
themselves, their friends and families and the whole community.<br />
Services which we provide to the community as a whole include<br />
talks, workshops and training around a range of drug related topics<br />
such as "safe disposal", "responding to overdoses" and "Cardio<br />
Pulmonary Resuscitation".<br />
During the year we received funding as part of the Local Drug<br />
Strategy for a Dual Diagnosis worker (Mental Health/ D&A) and as<br />
a result, Amanda (a psychologist) has joined the team, and we<br />
were also successful in our application to the National Illicit Drug<br />
Strategy for a Psychiatric Nurse, who will commence during 2004.<br />

• Primary Needle and Syringe Program<br />
This program provides health information, referral, advocacy, new<br />
injecting equipment and facilities for safe disposal of used<br />
injecting equipment.<br />
• Secondary Needle and Syringe Programs are located in each<br />
of our <strong>Community</strong> Health Centres, and provide a similar, but<br />
more limited range of services.<br />
• Walkdowns are a service specifically for residents of the High<br />
Rise Estates in <strong>Yarra</strong>, and provide a regular clean up of<br />
inappropriately disposed injecting equipment and other drug<br />
related paraphernalia.
Allied Health<br />

The Allied Health Team at NYCH is a dynamic multidisciplinary<br />
team that provides a wide range of health services and<br />
programs that aim to prevent illness, disease and injury and<br />
to promote good health and well-being.<br />
The Team consists of:<br />
Podiatry - our highly skilled Podiatrists do more than just cut<br />
toenails! We see people of all ages, including children, for the<br />
diagnosis, treatment, and prevention of disorders of the foot<br />
and lower leg. This might include biomechanical assessments,<br />
the making of splints or orthoses, surgery for ingrown<br />
toenails or wound management.<br />
Physiotherapy - our Physiotherapy team is available to see<br />
you for any issue you have that relates to pain or movement<br />
dysfunction. This might be due to problems that originate in<br />
the joints, muscles or nerves and could be the result of injuries,<br />
chronic diseases, pregnancy and childbirth or developmental<br />
issues to name a few examples. Our physiotherapists have a<br />
strong focus on exercise as this is a crucial part of any<br />
rehabilitation, and is also essential to good health.<br />
Nutrition - our Dietitians work with people to help them to<br />
choose and prepare food that is best for their health, lifestyle<br />
and budget. This might be because you have diabetes, heart<br />
disease, bowel problems, food allergies, appetite or stomach<br />
problems affecting your ability to eat, or just a need for cheap<br />
meals ideas.<br />
Occupational Therapy - our OT assists people to develop skills<br />
and strategies to change their environment and lifestyle, so<br />
they can live a fuller and more independent life. This can include<br />
an assessment of your home and provision of aids if necessary.<br />
Our OT service targets older adults and people of any age with<br />
a disability.<br />

Speech Pathology - provides assessment and treatment for<br />
preschool children with speech and language development<br />
problems. Some of the more common conditions that our<br />
Speech Pathologist helps with include stuttering, articulation<br />
problems and delayed language development.<br />
The Allied Health Outreach Team, consisting of an<br />
occupational therapist, dietitian, physiotherapist and podiatrist,<br />
provides services to people who are homeless or at risk of<br />
becoming homeless.<br />
The Allied Health Team is involved in a wide range of group<br />
programs and health promotion activities. Some of the<br />
highlights of the last year included:<br />
• The launch of "Eating Well Today, for Tomorrow" Paediatric<br />
Nutrition Needs Assessment<br />
• Celebrations with our local indigenous community including<br />
Family Day in the Park as a part of NAIDOC week, and a<br />
Christmas party<br />
• The establishment of a Youth Gym Program at the Collingwood<br />
High Rise Gym<br />
• The Hanen "It Takes Two to Talk" group program for parents<br />
with preschool children with delayed language development<br />
• The Better Health Program for people suffering from chronic<br />
diseases and arthritis<br />
• Tai Chi classes<br />
• The Living with Diabetes Sessions and the Be Well Expos<br />
This year we had some growth in Allied Health. This included<br />
an additional weekly session of dietetics and some additional<br />
hours in physiotherapy and podiatry to work with St Vincent’s<br />
Hospital in the Restoring Health Program. This program aims to<br />
strengthen the ability of the community sector to manage<br />
people with chronic diabetes, heart or lung disease, thereby<br />
decreasing inappropriate reliance on St Vincent’s Hospital.<br />
The Allied Health Team aims to provide a high quality,<br />
comprehensive range of services that are sensitive, relevant<br />
and responsive to the diverse social and cultural needs of all in<br />
our local community. We are always keen to hear of any ideas<br />
you may have of ways that we can improve our service.<br />
Dental Services<br />
Dental health continues to be an area of high need for the<br />
community, and, in common with other public dental services,<br />
we experience unacceptably long waiting lists.<br />
The additional funding announced in the State Budget was a<br />
welcome development, however a crucial issue remains the<br />
availability of dentists to work in the public sector. As part of our<br />
contribution toward the training of more public sector dentists, we<br />
have continued and increased our program of undergraduate dental<br />
student placements. As a result, students are exposed to<br />
community dentistry, gain experience in working with our clients<br />
and help to raise the profile of public dentistry.<br />
• Disposals - we collect and dispose of injecting equipment<br />
which has been placed in the Sharp Safes located in<br />
numerous places throughout the municipality, and conduct<br />
regular "sweeps" of parks and other public places where<br />
people may have disposed of syringes inappropriately.<br />
We also respond to the "Disposal Hot Line” (9417 5125)<br />
and ensure that anyone who finds a discarded syringe can<br />
call and have it safely removed.<br />
• Next Door is our Primary Health Care Service for people<br />
who inject drugs and provides peer education, nursing<br />
services, medical service (including pharmacotherapies)<br />
counselling and numerous other programs such as Art<br />
Therapy and Anger Management. During the past year,<br />
we had over 11 000 client contacts and 186 people<br />
commenced a pharmacotherapy (Methadone or<br />
Buprenorphine) program.<br />
• Holding it Together is one of the Hospital Admission Risk<br />
Programs that we deliver in conjunction with St Vincent’s<br />
Hospital, and targets people with drug issues who are<br />
frequent presenters in the emergency department.<br />
• <strong>Yarra</strong> Drug & Health Forum. The Executive Officer of the<br />
<strong>Yarra</strong> Drug & Health Forum is located with our Drug Safety<br />
Services, and the Forum continues to play an active role in<br />
community development and advocacy around drug issues.<br />
In an effort to maximise the available time and reduce waiting lists,<br />
this year we have contracted with a dental prosthetists, whose role<br />
is to make both full and partial dentures, and thus "free up" more<br />
dental time.<br />
Within the constraints of the funding, our dental team provides<br />
general and emergency dental care to holders of Health Care Cards<br />
and their dependent children. In addition, we now provide services<br />
to all pre school children that require dental care. (Some of these<br />
services attract a co payment, which is set by the Government, and<br />
is currently at $21.00 per visit, however, there is provision for "no<br />
cost" treatment for people with complex health needs and for all<br />
children of Health Care Card Holders).<br />
In addition we offer some specialist clinics for people who are<br />
homeless and for those with drug and alcohol issues, and these<br />
are in general provided at no cost.<br />
The importance which our community places on dental care was<br />
underscored during the year, when the Health and Wellbeing<br />
Committee of the Fitzroy Neighbourhood Advisory Board made<br />
arrangements for the residents of the Atherton Gardens High Rise<br />
Estate to have access to dental services at a reduced cost.<br />
We look forward to a productive year ahead, while we endeavour to<br />
offer optimum dental care to our community.

Health Promotion is a core component of all our programs and<br />
services and is defined as ‘the process of enabling people to<br />
increase control over, and to improve their health’ (Ottawa<br />
Charter, 1986). NYCH is committed to improving the Health of<br />
the community by incorporating Health Promotion principles<br />
into all aspects of our planning and service provision.<br />
NYCH has three priority Health Promotion areas:<br />
• Families & Children,<br />
• Chronic Illness and<br />
• Access to Services.<br />
A multidisciplinary approach is<br />
adopted when selecting strategies<br />
for addressing these issues.<br />
The Chronic Illness Group have<br />
implemented a "Eat Well, Move<br />
Well, Be Well" Expo at our Centres<br />
and at public housing estates<br />
across <strong>Yarra</strong>. (See the photo on the<br />
August Page). The Access Working Group is predominately<br />
involved in improving access to our services by particular<br />
communities that sometimes have difficulty accessing health<br />
services, with a particular focus on improving access for ATSI,<br />
Vietnamese and Refugee communities. The Families and<br />
Children’s Working Group has focussed on developing<br />
material in different languages to be distributed across the<br />
public housing estates in <strong>Yarra</strong>, and on developing and<br />
strengthening links with local primary schools and Maternal &<br />
Child Health Centres.<br />
The ‘Health Promotion in Public Housing’ project was<br />
re-funded during the year and aims to improve health<br />
promotion practice across the public housing estates of<br />
Collingwood, Fitzroy and Richmond. The program also has<br />
close links to the Neighbourhood Renewal Program and is an<br />
invaluable resource to the Health and Wellbeing Committees<br />
on the high rise estates. NYCH continues to be a key<br />
stakeholder in the Neighbourhood Renewal Program, with<br />
many staff involved in numerous activities.<br />
NYCH also provides a wide range of social, therapeutic,<br />
recreational and exercise based groups to cater for the needs<br />

The <strong>Community</strong> Liaison Committee (CLC) is one important<br />
element in our overall aim to empower the community and<br />
increase participation in society’s structures. The CLC has been<br />
a part of NYCH for nearly ten years, and its meetings are a real<br />
highlight on our calendar of events. There are usually over 120<br />
participants and the meetings are simultaneously translated<br />
into eight community language - so they are definitely not for<br />
the faint hearted! They provide an opportunity for community<br />
members to give us feedback on our services,<br />
receive new information, and most importantly to<br />
strengthen community and cross- cultural bonds.<br />
An offshoot of the CLC is the <strong>Community</strong><br />
Representatives Group, which includes one or two<br />
people from each of the NYCH communities, and<br />
enables more in-depth discussion than is possible at<br />
the large CLC meetings. The aim of the<br />
Representatives Group is to ensure that the voices of<br />

various communities are effectively heard by NYCH,<br />
and Representatives are asked to think of themselves<br />
as a conduit for information between their particular<br />
community and NYCH as a whole.<br />
Another important part of our work to strengthen participation<br />
and engagement has been the <strong>Community</strong> Empowerment<br />
Project (CEP). This project, funded by the Commonwealth<br />
Government and auspiced by the City of <strong>Yarra</strong>, was delivered in<br />
Collingwood, Richmond and Fitzroy. NYCH managed the<br />
Collingwood Project and was represented on the Fitzroy<br />
Project, (which was auspiced by St Mary’s House of Welcome).<br />
The CEP delivered accredited training to participants, and<br />
supported them with a variety of community strengthening<br />
projects. These included the establishment of a Sewing Group,<br />
the organisation of a Campaign against Centrelink Closure,<br />
Ballroom Dancing, Reconciliation BBQs, the Laughing Group,<br />
Photo Exhibitions and a whole host of other projects and<br />
activities developed by the CEP participants.<br />
The Collingwood CEP was supported by Kellie Nagle, and<br />
she did an outstanding job!<br />

The Reception, Finance and IT Teams are the people who keep the<br />
wheels of the organisation turning.<br />
Reception is well known to everyone who visits any of our centres,<br />
as it is usually a receptionist who will make an appointment, and<br />
provide information about our services. However, receptionists are<br />
also the people who maintain client records, file results, archive<br />
records which are no longer needed, and ensure that the waiting<br />
areas are welcoming and well organised.<br />
Our Finance staff keep track of the budget, ensure that we meet<br />
our accountabilities to the many funding streams which make up<br />
the budget, pay our creditors and send out invoices as required.<br />
The IT staff maintain and trouble shoot our IT system, provide<br />
reports on service usage, train staff in new applications and<br />
represent our needs to sector wide forums.<br />
Without these "front of house" and "back of house" teams, the<br />
clinicians who provide all the services which are so valued by the<br />
community could not do their work.<br />

Nursing<br />
The past 12 months have been an exciting time for the<br />
<strong>Community</strong> Health Nursing Team. Our nurses and midwifes<br />
continue to provide a high standard of clinical and community<br />
nursing services that are culturally appropriate, dynamic and<br />
sensitive to the needs in our community. The aim is to empower<br />
people and assist them to manage their health problems.<br />
The Nursing Team consists of:<br />
• Clinic Nurses, who undertake health checks such as blood<br />
pressure and blood sugar level, wound dressings or<br />
assistance with medication management - the flexible<br />
‘drop-in’ nursing clinics in Collingwood and Fitzroy are open<br />
Monday to Friday, and also on Saturday mornings in<br />
Collingwood. You can bring in your children for their<br />
scheduled immunisations, obtain travel vaccinations for<br />
yourself and the yearly flu injection for adults.<br />
Through the Well Women’s Clinics nurses provide<br />
information on all aspects of women’s health, contraception<br />
and pregnancy. You can come along and have your<br />
routine Pap Smear done. This year Lisa Harris has set up a<br />
weekly Well Women’s Clinic on the Atherton Garden<br />
Estate in Fitzroy and is available every Thursday morning.<br />
• The <strong>Community</strong> Midwives offer antenatal care, pregnancy<br />
education, and support women and their families before and<br />
after the birth of their child. The Midwives work closely<br />
with GPs and maternity hospitals to provide a holistic and<br />
integrated service for women.<br />
• Aged Care Nurse, Marlies Blatz, provides an outreach<br />
service to older people living at home, and support to their<br />
families. This includes home visiting and monitoring, health<br />
assessments, case planning and referrals. She also provides<br />
information on a broad range of aged care issues.<br />
• HARP Nurses The Hospital Admission Risk Program has<br />
been developed to assist people with chronic conditions<br />
to reduce the preventable use of hospital admissions.<br />
We are part of two separate projects which are part of this<br />
overall program - one involves children and young people<br />
under 18 who are affected by asthma, and one involves<br />
older people with chronic heart failure, respiratory<br />
illness and diabetes.<br />
All our <strong>Community</strong> Health Nurses also deliver care through<br />
health promotion and education activities to community<br />
groups. If you are interested in a nurse speaking to your group<br />
you can contact them through any of our Centres.<br />
Medical<br />
Our dedicated Doctors provide a high quality medical service to<br />
the community. We have medical practices at both Collingwood<br />
and Fitzroy, as well as a sessional practice at Carlton (with a<br />
focus on mental health issues) and a practice at Next Door, our<br />
service for injecting drug users.<br />
During the year both the Collingwood and Fitzroy practices<br />
were accredited through "GPA Accreditation Plus" a system of<br />
review and benchmarking which provides assurance to all our<br />
patients about the quality of our systems and processes.<br />
External reviewers conduct assessments that look at matters<br />
such as:<br />
• Access and availability<br />
• Communication with patients<br />
• Medical record systems<br />
• Risk reduction and disease prevention<br />
• Patients’ rights<br />
• Continuous education<br />

Both practices were assessed very highly through this process,<br />
and it’s great to get this kind of independent confirmation about<br />
the quality of the services that the practice as a whole provides.<br />
The Collingwood and Fitzroy practices are also accredited as<br />
Training Practices, which means that we offer 6-month rotations to<br />
General Practice Registrars, and give them the opportunity to<br />
experience working in a <strong>Community</strong> Health setting.<br />
One of the real advantages of locating a medical service within a<br />
<strong>Community</strong> Health setting is that the GPs can work collaboratively<br />
with other disciplines to provide an holistic response.<br />
For example, GPs work collaboratively with counsellors,<br />
caseworkers, nurses and other disciplines to ensure that we<br />
provide an integrated response to the complex health needs of<br />
refugees and asylum seekers.<br />
A further example of the integration that is made possible by the<br />
provision of medical services within a <strong>Community</strong> Health setting<br />
occurs at the Fitzroy Centre, where we have developed what is in<br />
effect a "centre of excellence" in the area of drug treatment.<br />
The continuum of service, which includes a Needle and Syringe<br />
Program, pharmacotherapies, dental services, dietetics, and other<br />
Allied Health Services is widely recognised as exemplary, and has<br />
resulted in numerous high level national and international<br />
delegations visiting us to learn from our models and practices.<br />
The flip side of our high quality and resultant "popularity" is that our<br />
GPs are increasingly booked well in advance, and long waiting<br />
times are becomingly increasingly common - we ask all our<br />
patients to be more "patient" when experiencing these delays.<br />
Pharmacy<br />
In Victoria there are only two Pharmacies located in <strong>Community</strong><br />
Health Centres, and we are fortunate to have one of those at our<br />
Collingwood Centre. The Pharmacy is funded by the Commonwealth<br />
Department of Health and Ageing, and during the last financial<br />
year we dispensed over 38 000 prescriptions to our clients.<br />
In addition to dispensing prescriptions the pharmacists provide<br />
information about medicines and counsel people on the correct<br />
use of their medications and related issues. The pharmacists also<br />
regularly talk to local community groups about effective use of<br />
medication and participate in campaigns such as "Be Wise with<br />
Medicines" and "Quit Week".<br />
The Pharmacy can be used by anyone with a prescription from one<br />
of the doctors at <strong>North</strong> <strong>Yarra</strong> <strong>Community</strong> Health and a pre-paid<br />
Pharmacy Contribution scheme is available for people who use the<br />
pharmacy services frequently.

over 15 000 group attendances during the year. Given that 41% of our<br />
clients were born outside Australia, it’s not surprising that 37% of our group<br />
attendances were for cultural groups, although many people from a variety<br />
of backgrounds also attend the other types of groups that we offer. The<br />
opening of the "Youth Space", at the base of the Collinwood High Rise has<br />
seen a big increase in our contact with young people, and now 10% of all<br />
group attendances are with young people.<br />

During the year we provided nearly 190 000 individual<br />
services across all our service types. This equates to<br />
over 3500 people a week coming into the centres,<br />
which explains why our waiting rooms can<br />
sometimes be "standing room only".<br />
Nearly a third of all contacts were in the area of Drug<br />
Safety Services, which includes our Needle & Syringe<br />
Programs, Drug and Alcohol Counselling, and a<br />
multiplicity of other services provided particularly at<br />
348 and 350 Smith Street.<br />
Our General Practitioners had nearly 40 000<br />
consultations during the year, while our pharmacy<br />

Overall, just over 10 000 registered<br />
clients utilised our services during the<br />
2003/04 year, about 500 more than in<br />
the previous year. These figures do<br />
not include the large number of<br />
unregistered clients who utilise our<br />
services, particularly our Drug Safety<br />
Services. About 55% of our clients<br />
are female, although in the 0-9 and<br />
the 40-49 age groups there is little<br />
difference in utilisation by males/<br />
females. In all other age groups<br />
however, our services are more<br />
utilised by women. This is particularly<br />
marked in the 20-29 age group (when<br />
a lot of women are giving birth) and in<br />
the 80+ age group, since women<br />
tend to live longer. While the overall<br />
number of clients utilising our<br />
services has increased, the proportion<br />
of male/female have stayed<br />
remarkably similar.

Our income for the 2003/04 year $6,972,525, and is derived from many<br />
sources. The Victorian Department of Human Services is our major funder,<br />
providing around 54% of all our funds through a variety of program areas.<br />
