Whole Foods Market ® Financial Aid Application
Financial assistance ($250 maximum) is provided on the basis of need and is only available to those students
living in the District 202 boundary. The deadline for application is Friday, May 24, 2013. The financial
aid committee reviews each application and makes decisions by Friday, June 7. For questions contact

INSTRUCTIONS
Complete and return the Camp Registration Form (page 19) with the required $15 minimum deposit.
Complete and sign the Financial Aid application below.
Have your child write a letter expressing why he/she wants to attend the camp, what he/she would
contribute, and how he/she would benefit.
Mail or hand deliver the above to the ETHS Athletic Office, 1600 Dodge Ave, Evanston, IL 60201-3449,
by Friday, May 24, 2013.
Please note, any application without completed registration form and the required $15 deposit will not be
processed. A refund will be processed if the application is denied or the family does not accept the award.
